恩度联合吉西他滨和顺铂治疗晚期肺鳞癌的临床疗效及对患者生活质量的影响

摘　　要：目的探讨吉西他滨联合顺铂一线治疗晚期肺鳞癌的临床疗效及对患者生活质量的影响。方法将60例晚期肺鳞癌患者随机分为2组,每组30例。治疗组患者釆用恩度持续静脉泵入联合吉西他滨与顺铂给药,而观察组患者仅采用吉西他滨联合顺铂给药;3周为1个疗程,每2个疗程进行疗效和不良反应评价。结果治疗组客观有效率和疾病控制率与观察组相比,差异无统计学意义(53.33%vs 33.33%,83.33%vs 73.33%,P>0.05)。2组患者的不良反应发生率接近,其中包括恶心呕吐、血液系统毒性和心脏毒性,经统计分析,2组差异无统计学意义(P>0.05)。疗程结束后,2组患者的认知功能、情感功能、角色功能、躯体功能、社会功能和整体健康状况的得分与治疗前相比,均显著升高(P<0.05);治疗组患者的改善程度显著优于观察组(P<0.05)。结论恩度持续静脉泵入联合吉西他滨与顺铂治疗晚期肺鳞癌时,临床疗效肯定,不良反应较少,且可明显改善患者生活质量,在临床上值得推广应用。Objective To study the clinical efficacy and its effect on quality of life of gemcitabine combined with cispla- tin in the first-line treatment for patients with advanced lung squamous carcinoma. Methods 60 patients with advanced squamous cell carcinoma were randomly divided into the treatment group and the observation group, each group with 30 patients. Patients of the treatment group were treated with endostar with continuous intravenous pump combined with gemcitabine and cisplatin, while patients of the observation group were treated with gemcitabine and cisplatin,3 weeks for a course. Evaluated the efficacy and ad- verse reactions every 2 courses. Results There were no significant differences in the objective effective rates and disease control rates between the 2 groups （53.33% vs 33.33% ,83.33% vs 73.33% ,P 〉0.05 ）. The incidence of adverse reactions of the 2 groups were similar,including nausea and vomiting, blood system toxicity and cardiotoxicity with no significant difference （P 〈 0.05）. After the treatment, the scores of the cognitive function, affective function, role function, physical function, social function and overall health status of the 2 groups were significantly higher compared with those before treatment （ P 〈 0.05 ）, and the treat- ment group was significantly better than the observation group （P 〈 0.05）. Conclusion Endostar administration with continuous intravenous pumping combined with gemcitabine and cisplatin can significantly improve the clinical efficacy of advanced lung squamous cell carcinoma and quality of life,with less adverse reaction, and can be applied in clinical practice.
