两种改良Millard法修复单侧唇裂的临床对比效果观察

摘　　要：目的对比分析两种改良Millard法修复单侧完全性唇裂唇高的恢复情况,为临床单侧完全性唇裂的治疗提供依据。方法选取先天性单侧唇裂患儿50例,随机分为2组,每组25例。第一组采用石冰的个体化改进MillardⅡ术式,第二组采用不做8点的改良Millard术式。两组患儿分别在手术后即刻和术后6个月随访,测量每名患儿健、患侧唇高并计算其差值,再进行两组对比。结果两组患儿手术完成即刻唇高差异无统计学意义(P=0.816);6个月后随访,两组间健-患侧唇高的比较差异有统计学意义(P=0.01)。结论在修复单侧完全性唇裂初期手术时,石冰的个体化改进MillardⅡ术式,应用范围更广泛,唇高改善理想;不做8点的改良Millard术式避免了鼻底的横行疤痕,但患侧唇高下降不足。Objective To compare the postoperative lip height recovery of the unilateral complete cleft lip repaired with two different types of modified Millard techniques,and to provide proofs for clinical treatment of this disease. Methods A total of 50 cases of pediatric patients with complete unilateral cleft lip were selected and investigated. Fifty cases were allocated into two groups randomly,with25 cases in each group. The first group accepted the individualized modified Millard Ⅱ technique improved by Shi Bing,and the second group adopted the modified Millard technique with ＂8 points＂being canceled. Two groups of pediatric patients were followed up immediately after surgery and 6 months after surgery,with their lip height in the healthy and ill sides measured,corresponding differences calculated,and a comparison between groups conducted statistically. Results Immediately after the surgery,there was no statistical difference of the lip height in pediatric patients between groups（P 〉0. 05）; after 6 months follow-up,statistical difference was found with respect to the difference of the lip height in the healthy and ill sides between groups（P〈0.05）. Conclusion In the early stage of unilateral complete cleft lip repairing,the individualized modified Millard Ⅱ technique improved by Shi Bing shows extensive range of application and satisfactory improvement of the lip height in the ill side; meanwhile,modified Millard technique with ＂8 points＂being canceled avoids the transverse scar of nasal bottom,but accompanies with an insufficient declining of the lip height in the ill side.
