摘  要:目的探讨中国汉族人群中AGT基因上的rs2067853和rs4762单核苷酸多态性与出血性脑卒中的相关性。方法研究采用病例-对照研究方式,收集出血性脑卒中患者共255例(ICH组),健康体检者共321例(NC组)。基因分型采用Sequenom飞行时间质谱平台进行检测,χ~2检验分析两组间等位基因和基因型的分布频率是否存在差异,并且按照性别进一步进行分层分析。多因素交互作用的影响采用Logistic多元回归分析。结果 ICH组和对照组rs4762等位基因和基因型分布频率比较差异无统计学意义(均P>0.05)。男性中,rs2067853在两组间的基因型分布频率差异有统计学意义,TT基因型是出血性脑卒中的危险因素(P=0.014,OR=9.053);而女性中两组间差异无统计学意义(P>0.05)。Logistic回归分析表明携带rs2067853 TT基因型的男性、高血压、血糖、糖化血红蛋白、体质量指数是出血性脑卒中的风险因素。结论rs4762位点多态性与出血性脑卒中易感无关。而rs2067853位点多态性与出血型脑卒中易感的相关性存在性别特异性:与男性出血性脑卒中易感相关,TT是风险因素;与女性出血性脑卒中易感无关。Objective To study the association of rs2067853 and rs4762 in AGT gene with susceptibility to hemorrhagic stroke in Han Chinese population. Methods Two hundred and fifty-five hemorrhagic stroke patients( ICH group) and 321 healthy subjects( NC group) from Jiangsu Province were investigated in a case-control study.Polymorphisms of rs2067853 and rs4762 were detected by the time of flight mass spectrometry platform of Sequenom.The frequency differences of the genotypic and allelic distribution between the hemorrhagic stroke group and control group were assessed with the Chi-square test. The subjects were further stratified by gender and then assessed the association. The effect of AGT genotypes and other risk factors of stroke to ICH were analyzed by Logistic regression.Results No significant difference in the distribution of rs4762 genotypic and allelic frequencies was found between ICH group and control group( all P 0. 05). The frequencies analysis of rs2067853 stratified by sex revealed that a significant difference was observed only in male,with the frequency of the TT genotype being higher in patients than in controls( P = 0. 014,OR = 9. 053),while there was no significant difference in female for rs2067853( P 0. 05). Logistic regression analysis showed that the TT genetype in male,hypertension,blood glucose,HbA1C and Body Mass Index were the risk factors of hemorrhagic stroke. Conclusions rs4762 of AGT gene is not associated with hemorrhagic stroke in Han Chinese population. The polymorphism of rs2067853 related to hemorrhagic stroke with a sex effect: the rs2067853 is associated with hemorrhagic stroke in male and TT genotype is considered as a risk factor,but there is no association in female.
