激励式心理干预对高龄产妇自我管理能力及分娩结局的影响

摘　　要：目的探讨激励式心理干预对高龄产妇自我管理能力及分娩结局的影响。方法选择2015年5月~2016年4月住院分娩高龄产妇94例为研究对象,采用随机数字表法分为观察组和对照组各47例。对照组采用围产期常规护理干预,观察组联合应用激励式心理护理干预,比较两组自我管理能力、产程时间、分娩结局等指标。结果观察组健康知识水平、自我概念、自护责任感、自物护理技能、自我管理能力总分均明显高于对照组(均P<0.05);第一产程、第二产程、第三产程、总产程均明显短于对照组(均P<0.05);阴道分娩率93.62%明显高于对照组78.72%,产后出血、新生儿窒息、胎儿宫内窘迫等并发症8.51%明显低于对照组23.40%(P<0.05)。结论激励式自我管理能力干预有助于提高高龄产妇自我管理能力,缩短产程时间,改善分娩结局。Objective To investigate the effect of incentive psychological intervention on self-management skills and delivery outcome of elderly parturient women.Methods A total of 94 cases of elderly parturient women who had institutional delivery from May 2015 to April 2016 was selected as the research objects,and divided into observation group and control group using the random number table method,with 47 cases in each group.Patients in control group were performed perinatal routine nursing intervention,and based on this,patients in observation group were performed incentive psychological nursing intervention.The self-management skills,duration of labor,delivery outcome and other indicators of patients in the two groups were compared.Results The aggregate score of health knowledge level,self-concept,self-care responsibility sense,self-nursing skills and self-management skills in observation group was significantly higher than that in control group（P〈0.05）;the first,second,third or total stages of labor was significantly shorter than that in control group（P〈0.05）;the rate of vaginal delivery was 93.62% and significantly higher than 78.72% in control group;the incidence rate of complications such as postpartum hemorrhage,neonatal asphyxia and fetal distress in uterus was 8.51% and significantly lower than 23.40% in control group（P〈0.05）.ConclusionIncentive psychological intervention can help improve the self-management skills of elderly parturient women,shorten the duration of labor and improve the delivery outcome.
