摘  要:目的:检测武汉地区乙型肝炎病毒的基因型与耐药突变基因,为临床治疗乙型肝炎患者提供实验室依据。方法:采用PCR-反向斑点杂交法对308例乙肝患者进行乙肝病毒基因型分析,同时检测拉米夫定耐药突变基因和阿德福韦酯耐药突变基因。结果:308例乙肝患者中检出B基因型173例,C基因型78例,B/C混合基因型6例,未检出51例。123例(39.94%)患者的HBV出现拉米夫定耐药,涉及3个位点4个突变型;34例(11.03%)出现阿德福韦酯耐药突变,涉及2个位点2个突变型。结论:武汉地区乙肝病毒存在较高的耐药基因突变率,对乙肝患者进行抗病毒治疗前和治疗过程中,应检测耐药基因是否出现突变。Objective:To investigate the distributions of genotyping and drug resistance mutations of hepatitis B virus in Wuhan.Method:Serum samples were collected from 308 patients with HBV in Wuhan and all the samples were detected by reverse dot blot about the genotyping and drug resistance mutations.Result:Of the 308 patients,173 samples were genotype B and 73 samples were genotype C.Only 6 samples were both genotype B and C.51 samples were undectected.Lamivudine resistant mutations which involving 4 mutations of 3 loci were detected in 123(39.94%)of 308 patients with HBV,while adefovir resistant mutations detected in 34(11.03%)which involving 2 mutations of 2 loci.Conclusion:There was high rate of hepatitis B virus resistance mutations in Wuhan,suggesting that the drug resistance mutations of hepatitis B virus in patients should be detected before and after anti-virus treatment.
