缺血性结肠炎与结肠癌CT表现对照研究

摘　　要：目的探讨缺血性结肠炎与结肠癌CT影像鉴别要点。方法 2012年6月至2016年8月我院收治并经肠镜、CT检查,临床确诊缺血性结肠炎86例中,随机抽取21例作为研究组;并随机抽取24例我院CT检查、手术病理证实进展期结肠癌24例作为对照组。统计学分析比较两组CT影像不同。结果研究组脾曲近侧发生率(3/21)低于对照组(11/24);研究组病变肠管长度(17.2±7.9)cm大于对照组(6.7±2.9)cm;研究组病变肠管厚度(9.2±2.7)mm小于对照组(14.1±6.4)mm;研究组病变肠管密度(25±11)Hu低于对照组(36±5)Hu;研究组肠系膜密度增高(10/21)、壁层腹膜增厚(6/21)、近端肠管扩张(1/21)、管腔狭窄程度(5.1±2.1)mm与对照组(16/24)、(2/24)、(5/24)、(6.0±2.0)mm差异均无统计学意义,P值分别为0.195、0.167、0.253、0.201。结论缺血性结肠炎的好发部位、肠管累及范围、肠壁增厚程度及肠壁密度与结肠癌有明显区别,而且前者多局限于结肠脾曲以下肠管。Objective To study the different points of CT images between the ischemic colitis and the colon cancer. Methods The ischemic colitis CT imaging of 21 cases who are randomized selected from 86 cases in our hospital from June 2012 to August 2016, were analyzed retrospectively and compared with the control group included 24 cases CT imaging of advanced colon cancer, All cases were confirmed by surgical pathology or colonoscopy. Re-sults The ischemic colitis incidence of left hemicolon （3/21） is lower than the control group （11/24）, P=0.023; the length of colon involved by the ischemic colitis lesions （17.2±7.9 cm） is greater than the control group （6.7±2.9 cm）, P =0.000;the thickness of colon wall involved by the ischemic colitis lesions （9.2±2.7 mm） is less than the control group （14.1 ±6.4 mm）, /^O.OC^^the lesions density of the ischemic colitis （25 ±11 Hu） is lower than the control group （36±5 HU）, P^O.Ol; the ischemic colitis group in the mesenteric lesions （10/21）, the peritoneal thickening （6/21）, the in-testinal canal expansion （1/21）,the degree of luminal stenosis （5.1 ±2.1 mm） is no significant difference with the con-trol group （16/24）, （2/24）, （5/24）, （6.0±2.0 mm）, P values were 0.195, 0.167, 0.253, 0.201 respectively. Conclusion The ischemic colitis in common location, the involving range, the degree of bowel wall thickening and the density of lesions wall has the obvious difference with the colon cancer.
