肿瘤标志物联合检测在胃癌中的诊断价值

摘　　要：目的 探讨血清肿瘤标志物癌胚抗原（CEA）、糖类抗原199（CA199）、糖类抗原724（CA724）单独及联合检测在胃癌中的诊断价值.方法 采用电化学发光法对165例胃癌患者（研究组）、60例胃黏膜肠上皮化生及不典型增生患者（对照A组）及60例慢性胃炎及健康者（对照B组）进行血清肿瘤标志物CEA、CA199、CA724测定,并对三组3种血清肿瘤标志物进行比较.结果 研究组3种血清肿瘤标志物水平均高于对照A组和对照B组,并对照A组3种血清肿瘤标志物水平均高于对照B组,差异有统计学意义.研究组与对照A组单独检测阳性率比较：CA199、CA724差异未见统计学意义,CEA单独检测阳性率大于对照A组,差异有统计学意义;研究组与对照B组单独检测阳性率比较,阳性率均明显升高,差异有统计学意义;对照A组与对照B组单独检测阳性率比较,阳性率均明显升高,差异有统计学意义;研究组与对照A组比较,3种肿瘤标志物联合检测的ROC曲线下面积均大于各单项检测,其中单独检测CEA曲线下面积最大,与联合检测比较差异未见统计学意义,CA199和CA724与联合检测比较差异有统计学意义.研究组与对照B组比较,3种肿瘤标志物联合检测的ROC曲线下面积均大于各单项检测,差异有统计学意义;对照A组与对照B组比较,3种肿瘤标志物联合检测的ROC曲线下面积均大于各单项检测,差异有统计学意义.结论 血清肿瘤标志物CEA、CA199、CA724联合检测对胃癌的诊断具有一定的临床价值,可作为一种辅助检查,有助于胃癌的早期发现.Objective To investigate the diagnostic value of separate and combined detection of the serum tumor markers carcinoembryonic anti（CEA）,carbohydrate atigen 199（CA199）,carbohy-arate atigen724（CA724）in early gastric cancer. Methods The serum tumor markers CEA,CA199,CA724 of the three groups were measured including the 165 patients with gastric cancer （research group）,60 ca-ses of gastric mucosa intestinal metaplasia and atypical hyperplasia （control group A）and 60 cases of chronic gastritis and healthy persons after physical examination （control group B）were measured,the three kinds of serum tumor markers in the three groups were compared. Results three kinds of serum tumor markers levels of research group were all higher than those of control group A and group B,and three kinds of serum tumor markers levels of group A were all higher than those of group B. The differ-ences were significant（P 〈 0. 05）. The positive rates of separate tests of research group were compared with group A,the differences of CA199,CA724 were not proved to be significant,while the positive rates of CEA was greater,and the difference was significant（P 〈 0. 05）. When the positive rates of separate tests of research group were compared with group B,they were significantly increased,and the differences were significant（P 〈 0. 05）. When the positive rates of separate tests of group A were compared with group B,they were significantly increased,and the differences were significant（P 〈 0. 05）. When the combined and separate detections of research group were compared with group A,the AUC of combined detection were larger than the separate detections of CEA,CA199 or CA724. In the separate detections, the AUC of CEA were the largest,which had no significant difference compared with combined detection, while the AUCs of CA199,CA724 were significant.
