咬合垂直距离的恢复对重度磨耗者咬肌疲劳耐性的影响

摘　　要：目的探讨恢复咬合垂直距离对后牙重度磨耗患者咀嚼肌疲劳疼痛耐性的影响。方法选取重度磨耗患者6名,确定合适垂直距离,制作并戴用垫12周,观察颞下颌关节及咀嚼肌适应性;记录患者垂直距离恢复前后强力咬牙时咬肌疲劳疼痛和终止咬合时间;监测咬肌血氧含量变化,分析主观感受和血氧监测结果相关性;记录疲劳耐受时间;分析相关时点改变,探究恢复垂直距离对重度磨耗者咀嚼肌疲劳疼痛耐性的影响。结果恢复垂直距离后主观疲劳疼痛时点延后(P<0.05);肌疲劳耐受时间延长(P<0.05)。结论重度磨耗者咬合垂直距离的恢复对延迟咀嚼肌疼痛发生时间、提高咬肌疲劳耐性有一定的积极意义。Objective To investigate the effect of VDO restoration on fatigue resistance capability of masseter muscles in patients with worn teeth. Methods Six patients with worn dentition wore removable splint dentures for three months after the suitable vertical dimension of occlusion was determined. The adaptability of TMJ and masticatory muscle was observed. The start time of muscle fatigue, muscle pain, tolerant time of pain and oxygenation of masseter muscle were recorded. Results The start time of muscle fatigue and muscle pain and the time of stopping biting were delayed after VDO restoration compared with those before restoration（ P 〈 O. 05 ）. The muscle fatigue tolerance time was delayed. Conclusion VDO restoration in patients with worn teeth may have positive effect on delaying the start time of muscle fatigue, muscle pain and elongate the muscle pain tolerance time.
