摘 要:目的利用CT血管造影(CTA)三维重建技术用于了解滋养细胞疾病动脉血供特点。方法入组2010年1月至2016年6月临床诊断为"滋养细胞疾病"且行CTA检查的患者共19例,并入组25例健康志愿者作为对照,构建三维模型,并观察子宫动脉、卵巢动静脉、子宫和病灶动脉血管网及形态等。结果 19例滋养细胞肿瘤患者和25例健康志愿者均成功构建三维重建模型。结果显示,滋养细胞疾病组共有17例患者出现子宫动脉的增粗、迂曲(89.4%),呈典型的子宫动静脉瘘改变,12例患者出现卵巢动脉或静脉显影(63.2%);而健康志愿者组未见子宫动脉或卵巢动静脉异常(P<0.05)。此外,亦观察到滋养细胞肿瘤病灶内部具有丰富血管网。结论基于CTA的数字化三维重建技术对滋养细胞肿瘤的辅助诊断、了解其动脉血供情况具有重要价值。Objective To explore the arterial blood supply features of gestational trophoblastic disease (GTD) by three-dimensional reconstruction models based on computed tomography angiography (CTA). Methods We obtained the CTA-data of 19 patients diagnosed with GTD between January 2010 to June 2016 and 25 healthy controls. After reconstruction, all models were reviewed for the morphology of uterine arteries, arteries and vein of the ovary, the uterus and the arterial network of the lesion. Results All CTA-data of the patients and controls were reconstructed successfully. The uterine arteries were augmented and tortuous in 17 GTD patients (89. 4%) with typical appearance of uterine arteriovenous malformation. Furthermore, abnormal arteries and vein of the ovary can be seen in 12 patients (63. 2% ). Arterial network rich in blood supply existed in nearly all GTD patients. However, none of these abnormities appears on healthy controls (P 〈 0. 05). Conclusions Three-dimensional reconstruction models based on CTA plays valuably in diagnosing GTD and understanding its arterial blood supplies.
