艾司西酞普兰联合乌灵胶囊治疗脑梗死恢复期合并抑郁临床研究

摘　　要：目的：探讨艾司西酞普兰联合乌灵胶囊治疗脑梗死恢复期合并抑郁临床疗效。方法：150例脑梗死恢复期合并抑郁患者,以随机分组法分为对照组（75例）和观察组（75例）,分别采用艾司西酞普兰单用和在此基础上加用乌灵胶囊辅助治疗;比较两组患者近期疗效,治疗前后HAMD评分,生活质量评分、IL-6、IL-23、TNF-α、NE及5-HT水平等。结果：观察组患者近期疗效显著优于对照组（P〈0.05）;观察组患者治疗后HAMD评分显著低于治疗前、对照组（P〈0.05）;观察组患者治疗后生活质量评分显著高于对照组、治疗前（P〈0.05）;观察组患者治疗后IL-6、IL-23及TNF-α水平均显著低于对照组、治疗前,差异有统计学意义（P〈0.05）;同时观察组患者治疗后NE和5-HT水平均显著高于治疗前、对照组（P〈0.05）。结论：艾司西酞普兰联合乌灵胶囊治疗脑梗死恢复期合并抑郁可有效缓解抑郁症状,提高日常生活质量,并有助于调节IL-6、IL-23、TNF-α、NE及5-HT水平。Objective： To investigate the clinical effects of escitalopram combined with Wuling Capsule in the treatment of patients with cerebral infarction in convalescence stage and depression. Methods： A total of 150 patients with cerebral infarction in convalescence stage and depression were randomly divided into control group（75 patients） with escitalopram used alone and observation group（75 patients） with Wuling Capsule on the basis of control group. And the clinical efficacy for short-term,the HAMD score,quality of life score,the levels of IL-6,IL-23,TNF-α,NE and 5-HT before and after treatment of both groups were compared. Results： The clinical efficacy for short-term of observation group was significantly better than that of the control group（ P 〈 0. 05）. The HAMD scores after treatment of observation group were significantly lower than those of the control group and before treatment（ P 〈 0. 05）. The quality of life score after treatment of observation group was significantly higher than that of the control group and before treatment（ P 〈0. 05）. The levels of IL-6,IL-23 and TNF-α after treatment of observation group were significantly lower than those of the control group and before treatment（ P 〈 0. 05）. The levels of NE and 5-HT after treatment of observation group were significantly higher than those of the control group and before treatment（ P 〈 0. 05）. Conclusion： Escitalopram combined with Wuling Capsule in the treatment of patients with cerebral infarction in convalescence stage and depression can efficiently relieve the symptoms of depression,improve the quality of daily life and be helpful to regulate the levels of IL-6,IL-23,TNF-,NE and 5-HT.
