摘 要:目的:探讨能谱CT在不同类型脑膜炎的诊断价值。方法:选取2016年4月-2017年7月在惠州市第三人民医院住院或门诊收治的各种类型颅内感染病人。化脓性脑膜炎26例,结核性脑膜炎28例,病毒性脑膜炎25例。探讨化脓性脑膜炎、结核性脑膜炎及病毒性脑膜炎不同ke V水平下的CT值、有效原子序数及碘-水浓度等能谱成像定量参数。结果:化脓性、结核性和病毒性脑膜炎CT能谱特征参数值差异具有统计学意义(P<0.05)。化脓性脑膜炎不同Ke V下CT能谱特征参数低于结核性和病毒性脑膜炎。随着能量升高,CT能谱特征参数下降,差异具有统计学意义(P<0.05)。碘含量及CT能谱衰减曲线斜率比较,化脓性脑膜炎最低,结核性脑膜炎最高。而不同类型脑膜炎有效原子序数比较,差异无统计学意义(P>0.05)。结论:能谱CT诊断不同类型的脑膜炎可提供不同的参数,为脑膜炎的诊断提供了良好的诊断依据,具有较高的诊断价值,可在临床上推广使用。Objective: To explore the diagnostic value of energy spectral CT in the diagnosis of different types of men- ingitis. The clinical data were selected from those with different types of intracranial infection who had received treatment in the outpatients and inpatients of the hospital between April 2016 and July 2017, among which 26 were with suppurative menin- gitis, 28 with tubercular meningitis and 25 with viral meningitis. The spectral characteristic parameters of CT value, effective atomic number and Iodine - water concentration under different keVs of suppurative meningitis, tubercular meningitis and viral meningitis were compared. Results : The difference in the CT spectral characteristic parameter values of purulent, tuberculous and viral meningitis was shown to be statistically significant ( P 〈 0. 05 ). The CT spectral characteristic parameter values under different KeV of suppurative meningitis were shown to be lower than those of tuberculous and viral meningitis. With the in- crease of the energy, the CT spectral characteristic parameters decreased, the difference was statistically significant (P 〈 0. 05 ). The comparison of content of iodine and CT spectrum attenuation curve were shown to be the lowest in suppurative meningitis and the highest in tuberculous meningitis. The comparison of effective atomic number in different types of meningitis showed no statistically significant difference ( P 〉 0. 05 ). Conclusion: The energy spectral CT can provide different parame- ters for the diagnosis of different types of meningitis, thus providing a good basis for the diagnosis of meningitis. It is of higher diagnostic va|ue, and therefore of value in clinical applications.
