摘 要:目的探讨视网膜冷凝联合小梁切除术及丝裂霉素对新生血管性青光眼(NVG)患者术后眼压及血清白细胞介素-6(IL-6)、血管内皮生长因子(VV-GF)、转化生长因子B1(TGF-β1)水平的影响。方法66例NVG患者按照随机数字表法分为对照组与研究组,每组33例。研究组采用视网膜冷凝联合小梁切除术及丝裂霉素治疗,对照组仅采用小梁切除术治疗,术后1~6个月进行随访。统计对比两组临床疗效、并发症发生率,并分析手术前后两组眼压及矫正视力、血清IL-6、VEGF、TGF-β1水平变化情况。结果研究组治疗有效率为93.94%(31/33),高于对照组的69.70%(23/33),差异有统计学意义(P〈0.05);术后1、6个月,两组眼压较术前降低、矫正视力较术前增高,且研究组眼压低于对照组,矫正视力高于对照组,差异有统计学意义(P〈0.05);术后6个月,两组血清IL-6、VEGF、TGF-β1水平较治疗前降低,且研究组低于对照组,差异有统计学意义(P〈0.05);研究组并发症发生率[12.12%(4/33)]与对照组[9.09%(3/33)]比较差异未见统计学意义(P〉0.05)。结论视网膜冷凝联合小梁切除术及丝裂霉素治疗新生血管性青光眼效果显著,可有效降低眼压及血清IL-6、VEGF、TGF-β1水平,抑制虹膜新生血管形成,提高治疗效果及视力,且具有安全性。Objective To investigate the effects of retinal coagulation combined with trabeculec- tomy and mitomycin on levels of postoperative intraocular pressure ( IOP), serum interleukin-6 ( IL-6 ), vascular endothelial growth factor (VEGF) and transforming growth factor-β1 (TGF-β1) in patients with neovascular glaucoma (NVG). Methods A total of 66 patients with NVG were divided into control group and study group according to the random table methods, with 33 cases in each group. The study group was treated by retinal coagulation combined with trabeculectomy and mitomycin, the control group was treated by trabeculeetomy only, all patients were followed up for 1 -6 months after operation. Clini- cal efficacy and incidence of complications were statistically compared between the two groups, and the intraocular pressure and corrected vision, levels of serum IL-6, VEGF and TGF-β1 were analyzed before and after operation. Results The effective rate of study group [ 93.94% (31/33 ) ] was higher than that of control group [ 69.70% (23/33) ], the difference was statistically significant ( P 〈 0. 05 ) ; 1 and 6 months after treatment, the intraocular pressures of the two groups both decreased, but corrected visual a- cuities of the two groups both increased, and intraocular pressure and corrected visual acuity of study group were significantly higher than those of control group (P 〈 0. 05 ) ; 6 months after operation, the lev- els of serum IL-6, VEGF and TGF-β1 were significantly lower than those before treatment, and those in study group were lower than those in control group, the differences were statistically significant ( all P 〈 0.05 ) ; there was no significant difference in incidence of complications between study group [ 12. 12% (4/33) ] and control group [ 9.09% (3/33) ], P 〉 0. 05.
