摘  要:目的 拟在河北邯郸永年县农村人群中探索代谢组学标志物与2型糖尿病(T2DM)发生风险的关系及其能否改善T2DM风险模型的预测能力.方法 以邯郸眼病队列研究(2006-2013年)为基础,设计巢式病例对照研究,随访新发209例T2DM患者为病例组,按性别年龄匹配对照组394例,应用条件Logistic回归探索代谢组学标志物与T2DM风险的关系.结果 校正年龄、性别、体质指数、腰围和糖尿病家族史后,血浆酪氨酸、亮氨酸、缬氨酸、丙氨酸、甘氨酸等均与T2DM发生风险相关,比值比(OR)[95%置信区间(95% CI)]=4.56(1.77-11.73)、5.73(1.80-18.25)、6.73(2.02-22.34)、5.89(2.26-15.36)、0.39(0.17-0.89)(模型2,均P〈0.05).进一步校正糖脂标志物后,只有酪氨酸和氨基酸组合与T2DM发生风险相关[OR(95% CI)=3.36(1.10-10.29)、1.11(1.02-1.22),模型3,均P〈0.05].在传统危险因子模型上加入氨基酸变量后,C统计量增加均〈0.005,全部重分类指数和大部分整体鉴别指数的改变无统计学意义(均P〉0.05).结论 血浆支链氨基酸、芳香族氨基酸、丙氨酸、甘氨酸等与T2DM发生风险有关,特别是酪氨酸与T2DM发生风险强相关,但是未能显著改善T2DM传统危险因子风险模型的预测能力.Objective To investigate the association of plasma amino acids and the risk of developing of type 2 diabetes mellitus (T2DM) and their prediction ability of T2DM in a rural Chinese population. Methods A nested case-control study was carried out in the Handan Eye Study conducted from 2006 to 2013.During follow-up,209 individuals developed diabetes,and 394 individuals matched for age and gender were identified as controls. Relationship of metabolic biomarkers to incident T2DM was assessed by conditional logistic regression. Results After adjusted for age, sex, body mass index (BMI), waist circumference, and family history of diabetes, the plasma concentrations of tyrosine, leucine, valine, alanine and glycine were significantly associated with incident T2DM, and the odds ratios (OR) [95% confidence interval(CI)] were 4.56 (1.77-11.73), 5.73 (1.80-18.25), 6.73 (2.02-22.34), 5.89 (2.26-15.36) and 0.39(0.17-0.89),respectively(Model 2,all P〈0.05).After further adjustment of fasting plasma glucose and triglyceride, only tyrosine and the combination of four-amino acids were still significantly associated with incident rate of T2DM,and the OR(95% CI)were 3.36(1.10-10.29)and 1.11(1.02-1.22),respectively (Model 3, both P〈0.05). None of these amino acids significantly improved the C-statistics and net reclassification improvement. Only the four-amino acids combination significantly improved the integrated discrimination improvement, but the net changes were not remarkable. Conclusions Levels of branched-chain and aromatic amino acids, alanine and glycine, particularly tyrosine, are significantly associated with incident T2DM. However, none of these amino acids significantly improves T2DM risk prediction.
