高危型HPV E6/E7 mRNA在宫颈病变筛查中的价值探讨

摘　　要：目的评估高危型HPV E6/E7 mRNA（HR-HPV E6/E7 mRNA）在宫颈病变筛查中的价值。方法选取福建医科大学附属第二医院2013年8月至2017年3月共412例同时具有组织病理学、细胞学、HPVDNA和hr HPV E6/E7 mRNA检测结果的患者进行分析。以组织学HSIL＋、细胞学ASCUS＋为阳性,比较三种检测方法的敏感性、特异性、阳性预测值、阴性预测值以及在各级别宫颈病变中的检测率进行统计学分析。结果hr HPV E6/E7 mRNA的敏感性、特异性、阳性预测值、阴性预测值均高于HPV DNA及细胞学,差异有统计学意义（P〈0.01）。三种检测方法中只有HR-HPV E6/E7 mRNA阳性率在LSIL组与HSIL＋组组间差异有统计学意义（P〈0.001）。结论高危型HPV E6/E7 mRNA在宫颈癌筛查中具有重要的价值。Objective To investigate the value of high-risk HPV E6/E7 mRNA in screening cervical lesions.Methods 412 patients with histopathology,cytology,HPV DNA and hrHPV E6/E7 mRNA test results were selected in The Second Affiliated Hospital,Fujian Medicinal University from August 2013 to March 2017.T he sensitivity,specificity,positive predictive value and negative predictive value of the three detection methods and the positive rate in cervical lesions were analyzed statistically with HSIL＋ and ASCUS＋ as standards.Results The sensitivity,specificity,positive predictive value and negative predictive value of hrHPV E6/E7 mRNA were higher than HPV DNA and cytology,with statistically significant differences（P 〈0.01）.There was statistically differences in hrHPV E6/E7 mRNA positive rate detected by the three methods between LSIL group and HSIL＋ group（P 〈0.001）.Conclusion The High risk HPV E6/E7 mRNA has great value in screening cervical lesions.
