MMP-2,MMP-9和E-cadherin在皮肤恶性黑素瘤中的表达及意义

摘　　要：目的检测基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)和上皮型钙黏附蛋白(E-cadherin)在皮肤恶性黑素瘤中的表达及在肿瘤侵袭转移过程中的作用。方法采用免疫组化SP法检测MMP-2、MMP-9和E-cadherin在32例恶性黑素瘤及10例色素痣中的表达。结果 MMP-2与MMP-9在恶性黑素瘤中阳性表达率分别为93.75%和87.50%,均明显高于色素痣,组间差异有统计学意义(P<0.01,P<0.01);MMP-2与MMP-9在有淋巴结转移者中阳性表达率均高于无淋巴结转移者(P<0.05,P<0.01)。E-cadherin在恶性黑素瘤中阳性表达率为21.88%,明显低于色素痣,组间差异有统计学意义(P<0.01);E-cadherin在有淋巴结转移者中阳性表达率低于无淋巴结转移者(P<0.05)。结论 MMP-2、MMP-9在恶性黑素瘤中均呈高表达,E-cadherin却高表达于色素痣,对它们的检测可能成为预测恶性黑素瘤转移和预后的重要指标。Objective In order to investigate the expression of MMP-2, MMP-9 and E-cadherin in skin malignant melanoma, and to explore its role in invasion and metastasis of skin malignant melanoma. Methods Thirty-two cases of malignant melanoma and 10 cases of melanocytic nevus were detected by immunohistochemistry technique for the expression of MMP-2, MMP-9 and E-cadherin. Results The positive ratios of MMP-2 and MMP-9 in malignant melanoma were93.75% and 87.50%, respectively, and they were significantly higher than those detected in melanocytic nevu（P〈0.01, P〈0.01）. Positive expression ratios of MMP-2 and MMP-9 in the patients with lymph node metastasis were obviously higher than those without lymph node metastasis, respectively（P〈0.05, P〈0.01）. The E-cadherin positive expression rate in malignant melanoma was 21.88%,and it was lower than that in melanocytic nevus（P〈0.01）. In the patients with lymph node metastasis,the positive E-cadherin expression rate was lower than that without lymph node metastasis（P〈0.05）. Conclusion MMP-2 and MMP-9 have high expression in skin malignant melanoma, but E-cadherin has high expression in melanocytic nevus.They would be some important markers for predicting metastasis and prognosis of skin malignant melanoma.
