摘 要:目的:探讨血清肿瘤标志物癌胚抗原(Carcino-embryonic antigen,CEA)、糖类抗原125(Carbohydrate antigen 125,CA125)、糖类抗原19-9(Carbohydrate antigen 19-9,CA19-9)及甲胎蛋白(α-fetoprotein,AFP)四项联合检测在卵巢癌诊断中的应用价值。方法:回顾性分析43例卵巢癌患者的临床资料,将其作为观察组,另取43例卵巢良性疾病患者作为对照组。采用放射免疫分析法检测CEA水平,以酶联免疫吸附法检测AFP、CA125及CA19-9水平。比较两组肿瘤标志物的表达水平;比较各项肿瘤标志物单独检测与联合检测的灵敏度、特异度及准确度。结果:观察组CEA、CA125、CA19-9、AFP表达水平均明显高于对照组,差异有统计学意义(P<0.05);四项联合检测灵敏度及准确度均明显高于单项检测,差异有统计学意义(P<0.05),四项联合检测特异度与单项检测差异无统计学意义(P>0.05)。结论:血清肿瘤标志物四项联合检测可有效提高卵巢癌诊断的敏感度和准确率。Objective To investigate application values of combined detection of serum tumor markers in early diagnosis of o-varian cancer, including carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), carbohydrate antigen 19-9 (CA19- 9) and alpha fetoprotein ( AFP). Methods: 43 ovarian cancer patients were used as observation group, and another 43 benign ovarian disease patients were selected as control group. Their clinical data were retrospectively analyzed. The CEA level was detected by radio-immunoassay ,and those of AFP, CA125, and CA19-9 were measured by enzyme-linked immunosorbent assay. The tumor markers expression levels in the two groups were compared, and the sensitivities, specificities and accuracies of single detection and combined detection in each tumor marker were compared. Results: The expression levels of CEA, CA125, CA19-9 and AFP in the observation group were significantly higher than those in the control group, and the differences were statistically significant (P〈0. 05). The differ-ences in the sensitivity and accuracy between single detection and combined detection were statistically significant ( P〈0.05) ; howev-er, that in the specificity was not statistically significant ( P〉0. 05). Conclusions : The combined detection of four serum tumor mark-ers can improve the diagnostic sensitivity and accuracy of ovarian cancer.【
