摘　　要：目的探讨人工肝支持系统对重症肝炎肝硬化患者肝脏储备功能的影响。方法采用回顾性对照研究方法,选取60例重症肝炎合并肝硬化患者,按照治疗方法不同分为对照组和观察组。对照组采用内科综合治疗,观察组在对照组的基础上,采用人工肝支持治疗,对两组患者肝功能指标及肝功能储备指标吲哚菁绿(ICG)15 min储留率、肝脏有效循环血量、终末期肝病模型评分进行比较。结果两组患者治疗前后丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、血清白蛋白(ALB)、血清总胆红素(TBil)及凝血酶源总活动度(PTA)、ICG 15 min储留率、肝脏有效循环血量、终末期肝病模型评分等指标比较,差异有统计学意义(P<0.05)。结论在综合治疗的基础上,对重症肝炎合并肝硬化患者实施人工肝置换治疗,为肝细胞的自身修复创造了一个有利环境,有助于改善患者的肝脏储备功能。Objective To explore the influence of liver reserve function by artificial liver support system for patients with severe hepatitis and cirrhosis. Methods By a retrospective case-control study,60 cases of patients with severe hepatitis and cirrhosis were analyzed. According to the different method of treatment, they were divided into control group and observation group. The control group were treated by medicine comprehensive treatment, while the observation group had implementation of artificial liver replacement therapy on the basis of the control group. Patients' indexes of liver function and ICG 15 min retention rate,liver effective circulating blood volume,model for end-stage liver disease score of two groups were compared. Results Before and after treatment,alanine aminotransferase(ALT),aspertate aminotransferase(AST),serum total protein,serum total bilirubin(TBil) and thrombin source total activity(PTA),ICG 15 min retention rate,liver indicators such as effective circulating blood volume,the model for end-stage liver disease score of two groups were compared,and the difference was statistically significant(P<0. 05). Conclusions On the basis of the comprehensive treatment, implementing artificial liver replacement therapy in patients with cirrhosis provide a favorable environment for liver cell repair, which improve the patient's liver reserve function.
