术前C反应蛋白与白蛋白比值对胃癌患者预后的影响

摘　　要：目的观察胃癌患者术前血清中C反应蛋白(CRP)与白蛋白(Alb)水平比值对预后的影响。方法检测130例胃癌患者术前血清中CRP与Alb水平,计算CRP/Alb(CAR),并据此将患者分为高CAR组(H组)与低CAR组(L组)。分析CAR与临床资料的关系,应用Kaplan-Meier曲线对生存率进行分析,应用Cox多元回归分析影响胃癌患者预后的因素。结果 H组患者肿瘤浸润深度更深、淋巴结转移率更高、TNM分期更晚、BorrmannⅢ、Ⅳ型所占比例更多;H组患者的总体生存率(19.72%)与无病生存率(14.08%)均显著高于L组患者(OS:35.59%;DFS:20.34%)(P<0.05);肿瘤侵犯深度、淋巴结转移数量、TNM分期与术前CAR是影响胃癌术后总生存期的独立危险因素。结论 CAR是影响胃癌患者预后的独立危险因素,术前CAR升高,提示预后较差。Objective To observe the influence of the ratio of CRP and albumin （Alb） on the prognosis of patients with gastric cancer before operation. Methods The serum levels of CRP and Alb in 130 patients were detected, and calculate the CRP/Alb （CAR） andcalculate thecut-off value of CAR. The patients were divided into high CAR group （H group） and low CAR group （L group）. The relationship between CAR and clinical data was analyzed, survival rate was analyzed by Kaplan-Meier curve, and Cox regression analysis was used to analyze the prognostic factors of gastric cancer patients. Results The preoperative CAR levels（0. 515±0. 027） in the H group were significantly higher than those in the L group（0. 223±0. 006）（P〈0.05）. Patients in H group had deeper tumor invasion, more lymph node me- tastasis, higher stage of TNM, and more proportion of Borrmann Ⅲand Ⅳ. The overall survival rate（19.72%） and disease-free survival rate（14.08%） in the H group were significantly higher than those in theL group（OS： 35.59%; DFS： 20.34 % ）（P〈0.05）. The depth of tumor invasion, the number of lymph node metastasis, TNM staging and preoperative CAR were independent risk factors affecting the overall survival of gastric cancer after surgery. Conclusion CAR is an independent risk factor for prognosis of patients with gastric cancer. Preoperative CAR elevation suggests poor prognosis.
