摘 要:目的系统评价结膜下注射氨基糖苷类抗菌药物治疗眼部感染的有效性和安全性。方法检索PubMed、EmBase、Cochrane Library、万方数据库和中国知网数据库,检索氨基糖苷类抗菌药物结膜下注射治疗眼部感染的临床研究,截至2017年3月。用系统评价的方法,对眼部感染治愈率、房水内浓度、注射后眼部疼痛发生率、结膜水肿发生率主要指标进行统计分析。结果最终共纳入18篇临床研究。结膜下注射氨基糖苷类抗菌药物对眼部感染治愈率可达100%,房水内可达到治疗浓度,无全身毒性发生,但注射后眼部疼痛发生率(39.62%)和结膜水肿发生率(72.58%)较高。结论结膜下注射氨基糖苷类抗菌药物治疗眼部感染的有效性较好,安全性有待进一步考察。Abstract: Objective To systematically evaluate the efficacy and safety of aminoglycoside antibiotics in the treatment of ocular infection through subconjunctival injection. Methods PubMed, EmBase, Cochrane li- brary, Wanfang and CNKI databases were searched from inception until March 2017 to investigate clinical trials about subconjunctival injection of aminoglyeosidc antimicrobials for the treatment of eye infection. Systematic evaluation was used to analyze the main indexes such as the cure rate of eye infection, the drug concentration in aqueous humor, the occurrence of eye pain after injection and the incidence of conjunctival edema. Results A total of 18 randomized controlled trials were enrolled in our study. The results showed that the cure rate of eye infection through sub- conjunctival injection of aminoglycoside antibiotics was excellent and an adequate aqueous humor concentration could be attained. No systemic toxicity was found, but the'incidences of eye pain (39. 62% ) and con- junctival edema ( 72.58% ):were high. Conclusion Subconjunctival injection of aminoglycoside antibiotics was effective. Considering the low quality of included studies, the safety of subeonjunetival injection of aminoglycoside antibiotics needs to be further verified.
