肌萎缩侧索硬化症患者的认知功能及其影响因素

摘　　要：目的探讨肌萎缩侧索硬化症(ALS)患者的认知功能及其影响因素。方法选取ALS患者53例为ALS组,健康体检者31例作为对照组。比较两组事件相关电位P300检测结果、汉密尔顿抑郁量表(HAMD)评分和ALS功能评分量表(ALS-FRS)评分,并分析P300的影响因素。结果 ALS组处于抑郁状态的患者占54.72%(29/53),高于对照组的22.58%(7/31);其ALS-FRS分值为(26.74±5.92)分。ALS组P300的N2和P3潜伏期长于对照组(P<0.05),而P3波幅小于对照组(P>0.05)。多元线性回归分析显示,ALS组患者P3、N2潜伏期与年龄、抑郁呈正相关(P<0.05),与ALS-FRS评分呈负相关(P<0.01);P3波幅与ALS-FRS评分呈正相关(P<0.01),与抑郁呈负相关(P<0.05)。结论 ALS患者存在认知障碍,其与患者年龄、FRS分值及抑郁状态相关。Objective To explore the cognitive function and its influencing factors of patients with amyotrophic lateral sclerosis（ ALS）. Methods Fifty-three ALS patients（ ALS group） and thirty-one healthy individuals who underwent physical examination（ control group） were enrolled. The event-related potential（ ERP）,P300,the scores of Hamilton Depression Scale（ HAMD） and Amyotrophic Lateral Sclerosis Function Rating Scale（ ALS-FRS） were compared between the two groups. And the factors influencing P300 were analyzed.Results The patients with depression state,with a ALS-FRS score of（ 26. 74 ± 5. 92）,accounted for 54. 72%（ 29/53） in the ALS group,more than those in the control group,accounted for 22. 58%（ 7/31）. Compared to the control group,the N2 or P3 latency of P300 increased,but the amplitude of P3 decreased in the ALS group（ P〈0. 05）. The multiple linear regression analysis showed that the latency of P3 or N2 positively correlated with age and depression（ P〈0. 05）,but negatively correlated with the ALS-FRS score in the ALS group（ P〈0. 01）.The amplitude of P3 positively correlated with the ALS-FRS score（ P〈0. 01）,and negatively correlated with depression（ P〈0. 05）.Conclusions Cognitive impairment exists in the patients with ALS,which is related to the patient＇s age,FRS score and depressive state.
