摘 要:目的探讨低分子右旋糖苷改善脑部微循环治疗急性期出血性脑血管病的临床效果,推动急性出血性脑血管疾病的治疗,提高患者存活率和存活质量。方法选取98例急性期出血性脑血管疾病患者为研究对象,随机分为观察组和对照组。对照组患者行常规治疗,观察组在对照组常规治疗的基础之上,给予低分子右旋糖苷静脉点滴治疗,每天500 ml,每天1次,连续2周为1个疗程。对比治疗前后两组患者的血液流变学指标(全血高切黏度、全血低切黏度、血浆黏度和血小板聚集率)、GCS评分情况、神经功能缺损情况和临床疗效。结果治疗结束后,观察组的全血高切黏度、全血低切黏度、血浆黏度和血小板聚集率均显著低于治疗前,也显著低于对照组患者,差异有统计学意义(P<0.05);两组患者治疗结束后的神经功能缺损评分均显著低于治疗前,且观察组治疗后的神经功能缺损评分显著低于对照组,差异有统计学意义(P<0.05),观察组治疗结束后的GCS评分显著高于治疗前,也显著高于对照组治疗后的GCS评分,差异均有统计学意义(P<0.05);观察组患者的基本治愈率和总有效率显著高于对照组患者,差异有统计学意义(P<0.05)。结论低分子右旋糖苷能够改善急性期出血性脑血管疾病患者的脑部微循环,对急性期出血性脑血管疾病有非常良好的治疗效果,值得在临床中推广使用。Objective To investigate the clinical effect of low molecular weight dextran on improving cerebral microcirculation in acute hemorrhagic cerebrovascular disease,promote the treatment of acute hemorrhagic cerebrovascular disease,and improve the survival rate and quality of life.Methods A total of 98 patients with acute hemorrhagic cerebrovascular disease were selected as study subjects and randomly divided into observation group and control group.Patients in the control group were treated routinely.The observation group was treated with intravenous drip of low molecular weight dextran on the basis of conventional treatment in the control group.500 ml daily was given once daily for 2 weeks for 1 course of treatment.Hemorheological parameters(whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity and platelet aggregation rate),GCS score,neurological deficit and clinical efficacy were compared between the two groups before and after treatment.Results After the treatment,the whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity and platelet aggregation rate in the observation group were significantly lower than those before treatment and in the control group,the difference was statistically significant(P<0.05).The scores of neurological deficits after treatment in both groups were significantly lower than those before treatment,and the scores of neurological deficits in observation group were significantly lower than those in control group(P<0.05).After the end of GCS score was significantly higher than before treatment,but also significantly higher than the control group after treatment GCS score,the difference was statistically significant(P<0.05); observation group patients with basic cure rate and total effective rate was significantly higher than the control Group patients,the difference was statistically significant(P<0.05).Conclusion Low molecular weight dextran can improve cerebral microcirculation in patients with acute hemorrhagic cerebrovascular disease
