东北地区绝经期慢性牙周炎患者牙周状况初析

摘　　要：目的探讨东北地区女性绝经期慢性牙周炎患者的临床特点及牙槽骨病损情况,为进一步的诊断和治疗提供临床依据。方法采用随机抽样的方法选取2013年2月-2014年2月就诊于中国医科大学附属口腔医院的绝经期慢性牙周炎患者女性50例。分别检测其牙周探诊深度(probing depth,PD)、临床附着丧失(clinical attachment loss,CAL)、菌斑指数(plaque index,PLI)、出血指数(bleeding Index,BI)及牙槽骨缺损的形态。结果本次研究中绝经期女性慢性牙周炎患者的平均年龄为(53.9±3.5)岁,随着绝经时间的增长、患者的PD、CAL、PLI、BI指数及牙槽骨缺损程度呈上升趋势,PD、CAL、PLI、BI指数及牙槽骨缺损程度与绝经时间均呈正相关(P<0.05);牙槽骨密度与绝经时间呈负相关(P<0.001),随着绝经时间的增长慢性牙周炎患者的牙槽骨密度呈下降趋势。结论绝经的时间可能会增加慢性牙周炎的炎症程度,定期进行牙周检查及及时的牙周治疗是防治女性绝经期慢性牙周炎的重要措施。Objective To evaluate the possible effects of menopause of women in Northeast China on the severity of periodontal dis- ease and bone loss, to provide reference for further diagnosis and the therapy of periodontal disease. Methods 50 women in meno- pause with chronic periodontitis, who were treated in Stomatological Hospital Attached to China Medical University from February 2013 to February 2014, were randomly selected. Oral and periodontal parameters, including plaque index, probing depth, bleeding on pro- hing and attachment loss were recorded. Results The mean age of this study was 53.9±3.5 years old. PD, CAL, PLI, BI indexes and alveolar bone defect were positively correlated with the time of menopause （P〈0.05） and were increasing with the increase of meno- pause. The alveolar bone density of patients with chronic periodontitis decreased with the increase of menopause time. The alveolar bone density was negatively correlated with the time of menopause （ P〈0.001 ）. Conclusion The time of menopause does appear to signifi- cantly influence the severity of periodontal disease and bone loss. Our results suggest that poor periodontal status may be associated with low bone mineral density of postmenopausal women. Regular periodontal check and timely treatment are the important measures to pre- caution against chronic periodontitis in menopause.
