超声引导下经皮微波消融治疗肝血管瘤疗效及安全性评价

摘　　要：目的探讨超声引导下经皮微波消融(MA)治疗肝血管瘤的疗效及安全性。方法选取60例肝血管瘤患者,根据不同手术方法,将其分为微波消融术组和开腹肝切除组,每组患者30例,评价对比两种治疗方案的疗效及安全性。结果术后微波消融术组ALT低于开腹肝切除组,AST高于开腹肝切除组,且两种术后均降低趋向正常范围,差异具有统计学意义(P<0.05),经过治疗后微波消融术组缓解率为80.00%,高于开腹肝切除组的70.00%,但是差异有统计学意义(P<0.05);微波消融术组患者的各项手术指标(手术时间、术中出血量、住院时间)均明显优于开腹肝切除组(P<0.05);微波消融术组不良反应发生率为6.7%,低于对照组的20.0%,差异具有统计学意义(P<0.05),患者随访半年后微波消融术组复发率为3.30%,开腹肝切除组复发率为20.00%,差异具有统计学意义(P<0.05)。结论超声引导下经皮微波消融治疗肝血管瘤治疗效果显著,并且创伤小,复发率低,具有一定安全性。Objective To evaluate the efficacy and safety of ultrasound-guided percutaneous microwave ablation （MA） in the treatment of hepatic hemangioma. Methods 60 patients with hepatic hemangioma were divided into microwave ablation group and open hepatectomy group according to different operation methods. 30 patients in each group, The efficacy and safety of the 2 regimens were compared. Results The level of ALT in the group of microwave ablation was lower than that of open hepa- tectomy group, the level of AST was higher than that of open hepatectomy group, and both of them decreased to the normal range, the difference was statistically significant （ P 〈 0.05 ）. In the microwave ablation group, the remission rate was 80.00%, which was higher than that of the laparotomy group （70.00%） ,but the difference was statistically significant （P 〈0.05）. The operative indexes of the microwave ablation group were superior to the laparotomy group, （ P 〈 0.05 ）. The incidence of adverse reactions in the microwave ablation group was 6.7% ,which was lower than that of the control group ,20.0% （P 〈0.05 ） , and the difference was statistically significant （P 〈 0.05）. The recurrence rate was 3.30% in microwave ablation group and 20.00% in open liver resection group （P 〈 0.05 ）. Conclusion Ultrasound-guided percutaneous microwave ablation for hepatic hemangioma is effective and safe, with less trauma and lower recurrence rate.
