七氟烷吸入麻醉与靶腔输注丙泊酚对老年消化道肿瘤术后认知功能及血液流变学的影响

摘　　要：目的分析七氟烷吸入麻醉与靶腔输注丙泊酚对老年消化道肿瘤患者术后认知功能及血液流变学的影响。方法以98例行消化道肿瘤择期手术的老年患者为研究对象,根据手术时间顺序分为观察组和对照组,每组49例,两组患者均行气管插管全身麻醉,对照组行七氟烷吸入麻醉,观察组行丙泊酚靶控输注麻醉。比较两组的手术及麻醉恢复情况、美国简易智能精神状态检查量表(MMSE)评分和术后认知功能障碍(POCD)发生率、手术前后血液流变学指标变化情况。结果观察组患者的睁眼时间、拔管时间和定向力恢复时间均显著短于对照组(P<0.05);手术后6 h,观察组患者的MMSE评分与术前比较,差异无统计学意义(P>0.05),但显著高于对照组(P<0.05),且术后6h POCD发生率显著低于对照组(P<0.05);术中及术后两组血液流变学指标比较,差异均无显著(P>0.05)。结论与七氟烷吸入麻醉相比,老年消化道肿瘤患者丙泊酚靶控输注麻醉后的麻醉苏醒质量更高,对术后认知功能影响更小,但对血液流变学指标的影响差异不大。Objective To investigate the influence of sevoflurane inhalation anesthetics and propofol target-controlled in- fusion in postoperative cognitive function and blood rheology of elderly patients with digestive tract cancer. Methods 98 cases of aged patients with digestive tract cancer were divided into the control group and the observation group,49 cases in each group. Maintenance types of anesthesia of the control group were sevoflurane inhalation, and the observation group were propofol target- controlled infusion. The operation situation and the quality of anesthesia recovery, MMSE scores and POCD incidence rate, hemo- rheology parameter before and after operation were compared between the 2 groups. Results The eye opening time, removal time and orientation recovery time of the observation group were significantly less than those of the control group ;6 h postoperation,the MMSE scores and POCD incidence rate of the observation group were significantly better than those of the control group; The differences in hemorheology parameters between the 2 groups were not significant. Conclusion Compared with sevoflurane inha- lation anesthetics,the quality of anesthesia recovery of propofol target-controlled infusion in elderly patients with digestive tract cancer are remarkable, and the cognitive function is better, and the influence in hemorheology parameters are similar.
