摘 要:目的分析胃癌患者菌群特征,探讨胃内菌群促进胃癌发生的机制。方法共纳入88例胃癌患者,黏膜标本采集于癌性病变部位及毗邻癌性病变的正常胃黏膜组织。通过荧光原位杂交分析细菌定植情况,通过实时荧光定量多聚核苷酸链式反应(real-time quantitative PCR,q PCR)检测胃黏膜细菌数量及Fusobacterium菌属丰度。同时运用高通量测序方法对6例患者进行胃黏膜菌群分析。结果细菌主要分布在胃黏液层;癌组织的总细菌数量与癌旁组织相比略低。高通量测序结果表明,癌组织中菌群的多样性、结构和群落类型与癌旁组织相比,差异无统计学意义(P>0.05)。菌群组成分析发现,癌组织中的Fusobacterium、Moryella和Porphyromonas菌属丰度均显著增加(P均<0.05)。q PCR分析进一步证实癌组织中Fusobacterium菌属数量明显升高。结论癌性病变部位的菌群特征未发生明显改变,但发现了丰度增多的细菌,其可能具有促进癌症发生的作用。它们在胃癌发生中的作用仍有待进一步研究。Objective To analyze the characteristics of bacterial flora in patients with gastric cancer,and to explore the characteristics of microbiota in gastric cancer. Methods A total of 88 patients with gastric cancer were enrolled in the study. Samples were collected from cancerous lesions and paracancerous gastric mucosa from each patient. The bacterial localization was analyzed by fluorescence in situ hybridization. The bacterial load and abundance of Fusobacterium in gastric mucosa were determined by real-time quantitative PCR( q PCR). Molecular analysis of microbiota in cancerous tissues from 6 patients were performed. Results The results showed bacteria were mainly scattered in the mucus layer of stomach. The total bacterial load in cancerous lesions was slightly lower than that in paracancerous gastric mucosa.Molecular analysis of microbiota in cancerous tissues showed that the diversity,structure and community types were not significantly changed compared with paracancerous tissues( P〈0. 05). Nonetheless,the abundances of Fusobacterium,Moryella and Porphyromonas in cancerous tissues were significantly increased( all P〈0. 05). Enrichment of Fusobacterium in cancerous tissues was further validated by q PCR analysis. Conclusion Characteristics of microbiota in cancerous lesions do not alter significantly. Enrichment of bacteria with potential cancer-promoting activities in cancer is identified. Their role in gastric carcinogenesis remains further investigations.
