不同手术方法治疗翼状胬肉的疗效及对泪膜稳定性的影响

摘　　要：目的对比翼状胬肉切除联合带蒂结膜瓣转移术治疗与翼状胬肉切除联合角膜缘干细胞移植术治疗翼状胬肉的疗效,及对患者泪膜稳定性的影响。方法选取2015年1月至2016年12月我院收治的50例翼状胬肉患者作为研究对象。采用随机数字表法将其分为A组与B组,其中A组25例接受翼状胬肉切除联合带蒂结膜瓣转移术治疗,B组25例接受翼状胬肉切除联合角膜缘干细胞移植术治疗,比较两组患者治疗效果及术前术后泪膜稳定性变化。结果 B组患者术后视力改善效果明显优于A组(P<0.05),A、B组患者术前SIT、BUT比较无显著差异(P>0.05),术后A、B组患者Schirmer I泪膜分泌量(SIT)、泪膜破裂时间(BUT)指标均较治疗前明显升高,P<0.05,具有统计学意义;但术后A组SIT、BUT指标与B组术后比较仍无显著差异,P>0.05,不具有统计学意义;B组术后并发症发生率以及6个月随访复发率明显低于A组(P<0.05)。结论翼状胬肉切除联合带蒂结膜瓣移植术与翼状胬肉切除联合角膜缘干细胞移植术均可有效治疗翼状胬肉,但联合角膜缘干细胞移植术对患者视力改善更为理想,具有临床应用及研究价值。Objetive To observe the effect of pterygium excision combined with conjunctival flap rotation and pterygium excision combined with limbal stem cell transplantation in treating pterygium. Methods 50 patients of pterygium treated in our hospital from January 2015 to December 2016 were selected. There were randomly divided into A group and B group. 25 cases in group A received pterygium excision combined with conjunctival flap rotation. 25 patients in group B received pterygium excision combined with limbal stem cell transplantation. Changes of tear film stability were compared between the two groups before and after surgery. Results Postoperative visual acuity in group B was obviously better than that of group A（P〈0.05）. Schirmer I test after operation in the two groups and break-up time （BUT） was significantly higher than that before operation Conclusion Either pterygium excision combined with conjunctival flap rotation or pterygium excision combined with limbal stem cell transplantation can be effective treating pterygium. But patients undergoing pterygium excision with limbal cell transplantation got a better postoperative visual acuity.
