摘　　要：[目的]探讨城市社区老年高血压病人延续性护理的需求现状和相关影响因素。[方法]采用自制问卷对160例社区60岁以上老年高血压病人进行调查研究。[结果]居住情况和医疗费用支付方式对老年高血压病人延续性护理需求的影响有统计学意义(P<0.05);需求内容主要是健康教育、疾病相关指导和康复指导;需求方式包括专科护士服务、门诊复查、电话随访、网络通信、社区护士服务等。[结论]城市社区老年高血压病人对延续性护理的需求较为强烈,应针对服务人群的特征以适宜的方式提供延续性护理服务。Objective:To probe into the needs of elderly patients with hypertension on transitional care in urban community and its influencing factors.Methods:A total of 160 elderly hypertension patients above 60 years old were investigated by using a self-made scale.Results:There were statistically significance in effect of living conditions and medical expenses payment on transitional care needs of elderly hypertensive patients(P<0.05).The contents of need mainly included health education,disease related guidance and rehabilitation guidance.The styles of need included nurse services,out-patient review,telephone follow-up,network communication,community nurse services and so on.Conclusions:The needs of elderly patients with hypertension in urban community on transitional care were stronger.In view of the characteristics of the service population,the development of continuous nursing model to integrate service content and style was the guarantee of service quality.
