胰头局灶性脂肪浸润影像学表现

摘　　要：目的探讨胰头局灶性脂肪浸润影像学表现特征。方法回顾性分析7例胰头局灶性脂肪浸润患者的影像学资料,5例行CT平扫及增强扫描,其中3例行MRI检查,2例仅行CT增强扫描,所有病例随访8-30个月。结果 7例胰腺局灶性脂肪浸润病变位于胰头及钩突前部。5例局灶性脂肪浸润区域CT平扫平均值为11.4 HU,3例CT值位于10-30 HU区间,2例CT值位于-10-10 HU区间,增强扫描病灶强化程度均低于正常胰腺实质,胰头前部脂肪浸润区域与后部正常胰头实质有明显分界线。3例局灶性脂肪浸润区域MRI检查在同相位T1WI序列均呈等信号,反相位T1WI序列为中度到明显信号衰减。所有随访病例影像学检查脂肪浸润区域无明显变化。结论胰头局灶性脂肪浸润有一定的CT影像特点,MRI检查梯度回波同反相位序列具有准确的定性诊断价值。Objective To explore the imaging features of focal fatty infiltration in the head of the pancreas. Methods Retrospectively analyzed image findings of 7 cases with focal fatty infiltration in the head of the pancreas. 5 patients un- derwent both non-enhanced and contrast-enhanced CT scan,including MR imaging in 3 patients. 2 patients only performed enhanced CT scan. Follow-up CT or MRI scans were obtained in all patients at 8 - 30 months after the initial examinations. Results Focal fatty infiltration was dominantly located at the anterior aspect of the pancreatic head and uncinate process in 7 Patients. The average value of plain CT in 5 cases of focal fatty infiltration in the head of pancreas was 11.4 HU, CT val- ue of 3 cases is was in the 10 - 30 HU range, CT value of 2 cases is in the - 10 - 10 HU interval. Focal fatty infiltration is seen as a region of decreased attenuation in the pancreas on post-contrast CT. The interface between focal fatty infiltration in the anterior aspect of the head and the posterior aspect of the pancreatic head has a sharp demarcation. On MRI, regions of focal fatty infiltration are isointense relative to the surrounding pancreatic parenchyma on in-phase T1 -weighted images, and these regions show moderate to marked signal intensity loss on opposed-phase gradient-echo images in 3 patients. Focal fatty infiltration is stable on follow-up imaging examination. Condusion The CT features of focal fatty infiltration in the head of the pancreas is definite, MRI sequence can make a correct diagnosis.
