摘 要:目的比较宫颈鳞癌及癌前病变的活检标本中树突状细胞(DC)的数量及分布,以此评估局部细胞免疫在阻止宫颈鳞状上皮内病变进展中的作用。方法第二代杂交捕获(HC2)技术检测宫颈脱落细胞中HR-HPV的DNA,以HC2阴性的慢性宫颈炎病例(n=20)为对照组,试验组为HC2阳性病例:包括低级别鳞状上皮内病变(LSIL)(n=29)、高级别鳞状上皮内病变(HSIL)(n=46)及鳞状细胞癌(SCC)(n=20);通过免疫组织化学方法检测DC表型抗原S-100、CDla及CD83在各组中的表达。结果 (1)S-100和CD1a的阳性表达率依次为SCC>HSIL>LSIL>慢性宫颈炎,SCC、HSIL与慢性炎,SCC与LSIL相比差异有统计学意义(P<0.05);(2)CD83的阳性表达率依次为HSIL>LSIL>SCC>慢性宫颈炎,HSIL组与其余各组相比差异有统计学意义(P<0.05);(3)各组中S-100与CD1a表达呈正相关,r=0.69(P<0.01)。结论在宫颈SIL进展的过程中,S-100、CDla及CD83表达均逐渐升高,说明局部细胞免疫反应被激活;但SCC中CD83表达降低,反映成熟DC数量减少,说明在SCC中有效的细胞免疫功能被抑制。Objective To compare the number and distribution of dendritic cells(DCs) in biopsy specimens of cervical squamous carcinoma and precancerous lesions,and to evaluate the effect of local cellular immunity in preventing the progression of cervical squamous intraepithelial lesion.Methods HC2 technique was used to determine the DNA of HR-HPV in exfoliated cells of cervix.Chronic cervicitis cases with negative HC2(n=20) were included as the control group,and the cases with positive HC2 were included as the study group,including LSIL(n=29),HSIL(n=46) and SCC(n=20).Immunohistochemistry was used to determine the expression of DC phenotype antigens S-100,CDla and CD83 in each group.Results(1) The positive rates of S-100 and CD1 a were SCCHSILLSILchronic cervicitis.There were statistically significant differences between SCC vs HSIL vs chronic inflammation,and SCC vs LSIL(P0.05).(2) The positive expression rate of CD83 was HSILLSILSCCchronic cervicitis,whereas there were statistically significant differences between the HSIL group and other groups(P0.05).(3)There was a positive correlation between the expression of S-100 and CD1 a in each group,r=0.69(P0.01).Conclusion During the progression of cervical SIL,the expressions of S-100,CDla and CD83 gradually increase,indicating that the local cellular immune response is activated.However,the decrease of CD83 expression in SCC reflects a decrease in the number of mature DCs,suggesting that effective cellular immunity is inhibited in SCC.
