摘  要:目的探讨血清淀粉样蛋白A(SAA)、视黄醇结合蛋白(RBP4)、单核细胞趋化蛋白1(MCP-1)与缺血性脑卒中脑损伤程度及脑梗死体积的关系。方法 2015年10月至2016年10月选取收治的120例缺血性脑卒中患者为研究对象,另选取100例健康体检者为对照组。应用ELISA法测定缺血性脑卒中患者入院时、入院第1天、第3天、第5天、第7天血清SAA、RBP4、MCP-1水平,对照组测定体检当天血清SAA、RBP4、MCP-1水平。再根据缺血性脑卒中患者预后分组,测定两组患者血清SAA、RBP4、MCP-1水平。应用神经功能缺损程度评分(NHISS)对缺血性脑卒中患者神经损伤程度进行评价,应用CT观察患者脑组织梗死体积。应用Pearson单因素分析血清SAA、RBP4、MCP-1与缺血性脑卒中脑损伤程度及梗死体积的关系。应用受试者工作曲线(ROC)分析血清SAA、RBP4、MCP-1在缺血性脑卒中诊治中的应用价值。结果缺血性脑卒中组入院时血清SAA、RBP4、MCP-1水平显著高于对照组(P<0.05)。与入院时及入院第1天相比,缺血性脑卒中患者入院第3天、第5天、第7天血清SAA、RBP4、MCP-1水平显著下降(P<0.05)。与存活组比较,死亡组血清SAA、RBP4、MCP-1水平显著升高(P<0.05)。经Pearson单因素分析,缺血性脑卒中患者NHISS评分、脑梗死体积与血清SAA、RBP4、MCP-1水平呈正相关(P<0.05)。经ROC曲线分析,血清SAA、RBP4、MCP-1曲线下面积(AUC)分别为0.756,0.785,0.765。结论血清SAA、RBP4、MCP-1与缺血性脑卒中脑损伤及梗死体积呈正相关,通过测定缺血性脑卒中患者血清SAA、RBP4、MCP-1水平将有助于患者病情进展及预后的评价。Objective To investigate the relationship between serum amyloid A(SAA),retinol binding protein(RBP4),mooneye chemoattractant protein-1(MCP-1) and cerebral infarction severity.Methods 120 cases of patients with ischemic stroke admitted to the department of neurology were selected as the study group from October 2015 to October 2016.Another 100 cases of healthy subjects were selected as the control group.The serum levels of SAA,RBP4 and MCP-1 on the 1 st day,the 3 rd day,the 5 th day and the 7 th day after hospital admission were detected by enzyme-linked immunosorbent assay.The serum SAA,RBP4 and MCP-1 levels on the day of physical examination were measured in the control group.According to prognosis of ischemic stroke patients group,serum SAA,RBP4,MCP-1 levels were measured in two groups.The degree of neurological injury in patients with ischemic stroke and infarct volume of brain tissue were evaluated with Neurological deficit score(NHISS) and observed by CT.The relationship between serum SAA,RBP4,MCP-1 and the degree of brain injury and infarct volume of ischemic stroke were analyzed by Pearson univariate analysis.The application of serum SAA,RBP4,MCP-1 in ischemic stroke was analyzed by ROC.Results The levels of SAA,RBP4 and MCP-1 of ischemic stroke group were significantly higher than those in the control group(P <0.05).The levels of serum SAA,RBP4 and MCP-1 in patients with ischemic stroke were significantly lower than those on the 3 st day,the 5 th day and the 7 th day after admission(P < 0.05).The levels of serum SAA,RBP4,MCP-1 in the dead group were significantly higher than those in the survival group(P < 0.05).The levels of SAA,RBP4 and MCP-1 were positive correlation with NHISS score and cerebral infarct volume in patients with ischemic stroke(P < 0.05).The area under the curves of SAA,RBP4 and MCP-1 were 0.756,0.785 and 0.765 respectively by ROC curve analysis.Conclusion Serum SAA,RBP4 and MCP-1 levels are positively correlated with the degree of brain injury and infarction in ischemic stroke.The leve
