腹腔镜下保留盆腔自主神经的广泛性子宫切除术治疗早期宫颈癌的手术参数、术后膀胱功能和生存情况分析

摘　　要：目的研究分析腹腔镜下保留盆腔自主神经的广泛性子宫切除术(LNSRH)治疗早期宫颈癌的手术参数、术后膀胱功能和生存情况。方法回顾性分析2015年1月至2016年1月期间收治的107例宫颈癌患者资料。根据不同的术式,分为LNSRH组54例和普通腹腔镜下广泛子宫切除术(LRH)组53例。比较两组患者的手术参数、术后膀胱功能恢复情况及术后1年生存情况。结果两组患者的手术时间等各项手术参数差异均无统计学意义(P>0.05);LNSRH组的术后尿管留置时间、膀胱功能障碍、尿潴留及腹压排尿等情况均优于LRH组,差异具有统计学意义(P<0.05);两组患者术后1年的宫颈癌复发率均为1.9%,LNSRH组患者未出现死亡病例,而LRH组仅有1例,差异均无统计学意义(P>0.05)。结论 LNSRH与LRH治疗早期宫颈癌均有较好的疗效,但LNSRH能够更好地保护膀胱功能。Objective To study and analyze the operation parameters,postoperative bladder function and survival conditions of laparoscopic nerve-sparing radical hysterectomy(LNSRH) in the early stage of cervical cancer.Methods The data of 107 patients with cervical cancer treated in our hospital from January 2015 to January 2016 was retrospectively analyzed.According to different operation methods,they were divided into LNSRH group(n = 54) and general laparoscopic hysterectomy(LRH)(n = 53),and the operation parameters,the postoperative bladder function and survival condition were compared.Results The difference between operation parameters of two groups,such as operation time,were not statistically significant(P > 0.05);The indweling catheter time,bladder dysfunction rate,urinary incontinence,abdominal pressure urination,urinary retention of LNSRH group were better than those of LRH group,and the difference were statistically significant(P < 0.05);The postoperative cervical cancer recurrence rates were both about 1.9%,and there was no one in LNSRH group,and there was 1 death in LRH group,and the differences were not statistically significant(P > 0.05).Conclusion LNSRH and LRH both have good curative effect for early cerical cancer,but LNSRH can protect the bladder function better.
