治疗相关血液恶性肿瘤13例分析并文献复习

摘　　要：目的总结及探讨与治疗相关的髓系肿瘤及淋巴瘤的临床特点。方法回顾2012年1月至2017年2月13例治疗相关血液系统恶性肿瘤患者的临床资料,并结合文献进行分析。结果 13例患者于原发肿瘤确诊后均接受化疗或放化疗联合治疗。其中治疗相关髓系肿瘤(t-MN)9例,1例治疗相关急性淋巴细胞白血病(ALL),3例治疗相关非霍奇金淋巴瘤(NHL)。2例患者疾病快速进展,治疗无效,余11例经过诱导达完全缓解。结论治疗相关的血液恶性肿瘤经传统治疗后预后较差,因其伴有高龄、脏器功能差、既往化疗蓄积剂量和不良染色体核型和基因突变,故需采用新药抗甲基化治疗或者采用较积极的自体或者异基因移植,可改善治疗效果。Objective To explore and review therapy-related marrow series tumor and lymphoma in this hospital after conventional chemoradiotherapy. Following with the increasingly successful management of malignancies. overall survival period of primary malignancy appears to be prolonged and emerging data highlight therapy-related secondary tumor as well. The therapy-related myeloid tumors（ t MN） and therapy-related lymphomas in this department in recent years. Methods The clinical data of 13 patients with therapy-related hematological malignancies in this hospital during January 2012 to February 2017 were retrospectively analyzed. Results All these 13 patients were previously treated with chemotherapy or radiotherapy. Among them,9 patients were suffered with t-MN,1 case with therapy-related acute lymphoblastic leukemia（ ALL） and 3 cases with therapy-related non-Hodgkin＇s lymphoma（ NHL）. The course of disease in two cases was rapidly progressed and 11 cases achieved complete remission after induction treatment. Conclusion The prognosis of therapy-related hematological malignancies treated with conventional therapy is uniformly poor due to old age,poor function of organs,accumulation of previous chemotherapeutic dosages and complex karyotype chromosome and gene mutation. It needs antimethylation treatment or by use of autologous or allogeneic transplantation to improve the efficacy of treatment.
