基底样乳腺癌与三阴性乳腺癌的临床病理分析

摘　　要：目的分析基底样乳腺癌与三阴性乳腺癌的临床病理特征。方法回顾性分析54例三阴性乳腺癌患者的临床资料,根据免疫组化染色结果,筛选出29例基底样乳腺癌;分析三阴性乳腺癌、基底样乳腺癌的临床病理特点、组织学特点,对比三阴性乳腺癌中基底样乳腺癌与非基底样乳腺癌的组织学分型及分级,并分析CK5/6、CK14用于从三阴性乳腺癌中筛选基底样乳腺癌的临床价值。结果三阴性乳腺癌与基底样乳腺癌具有相似的临床病理特点,如淋巴结转移率、肿瘤直径、发病部位,及组织学特点,差异无统计学意义(P>0.05);三阴性乳腺癌中基底样乳腺癌与非基底样乳腺癌的组织学分型及分级亦相似,但基底样乳腺癌Ⅱ级、Ⅲ级非特殊浸润性癌比例显著高于非基底样乳腺癌,且非基底样乳腺癌Ⅱ级、Ⅲ级浸润小叶癌比例显著高于基底样乳腺癌,差异具有统计学意义(P<0.05);基底样乳腺癌高度表达CK5/6、CK14,利用CK5/6、CK14单独从三阴性乳腺癌中筛选基底样乳腺癌分别占75.86%、79.31%,差异无统计学意义(P>0.05);两者联合使用占96.67%,且CK5/6与CK14呈正相关(r=0.541,P<0.05)。结论基底样乳腺癌与三阴性乳腺癌的临床病理特征大部分重叠,CK5/6、CK14可作为从三阴性乳腺癌中筛选基底样乳腺癌的标记物。Objective To analyze the clinicopathological features of basal-like breast cancer(BLBC)and triple-negative breast cancer(TNBC).Methods Clinical data of 54 patients with TNBC were analyzed retrospectively.According to the results of immunohistochemical staining,29 cases of BLBC were screened out.Clinicopathological and histological features of BLBC and TNBC were analyzed,and histological type and classification of BLBC and non-basal-like breast cancer(NBLBC)were compared.In addition,the clinical values of CK5/6 and CK14 in screening BLBC were evaluated.Results There were no significant differences between TNBC and BLBC in the clinicopathological features,such as lymph node metastasis rate,tumor diameter,pathogenic site and histological characteristics(P>0.05).BLBC and NBLBC had similar histological type and classification.However,the proportion of grade Ⅱ and Ⅲ non-specific invasive cancer in BLBC was higher than that in NBLBC(P<0.05),and the proportion of gradeⅡandⅢinvasive lobular cancer in NBLBC was higher than that in BLBC(P<0.05).Both CK5/6 and CK14 were highly expressed in BLBC.The detection rate of CK5/6 for BLBC(75.86%)was not significantly different from that of CK14(79.31%)(P>0.05).The detection rate of combined CK5/6 and CK14 for BLBC was 96.67%,and CK5/6 was positively correlated with CK14(r=0.541,P<0.05).Conclusion The BLBC and TNBC have overlapping clinicopathologic features.The CK5/6 and CK14 can be used as the markers for screening BLBC from TNBC.
