摘 要:目的观察放射性肝损伤的病理形态学变化及其与剂量效应的关系,以及影响放射性肝损伤的发生因素,为肝癌的放射治疗提供参考依据。方法通过制备兔半肝放射性损伤动物模型,观察放射性肝损伤肝组织细胞因子——肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、转化生长因子-β1(transforming growth factor-β1,TGF-β1)、血小板源生长因子(platelet-derived growth factor,PDGF)、结缔组织生长因子(connective tissue growth factor,CTGF)的定位和表达情况。结果照射后,TNF-α、TGF-β1、PDGF、CTGF均显著表达于肝脏纤维间隔、中央静脉及汇管区周围细胞的胞质中。随着照射剂量的增加,不同照射剂量组肝脏相同取材区TNF-α、TGF-β1、PDGF、CTGF的表达水平进行性升高,差异具有显著性(P<0.05),且与对照组比较亦具有显著差异(P<0.05);相同照射剂量组肝脏不同取材区TNF-α、TGF-β1、PDGF、CTGF的表达水平随未照射区、低剂量区、高剂量区受照射剂量的增加进行性升高,差异具有显著性(P<0.05),且与对照组比较亦具有显著差异(P<0.05)。总剂量与TNF-α、TGF-β1、PDGF、CTGF的表达水平存在显著的正相关性(r=0.899,0.958,0.959,0.968,P<0.05),与胶原纤维含量(晚期放射性肝纤维化程度)也存在显著的正相关性(r=0.967,P<0.05)。TNF-α、TGF-β1、PDGF、CTGF表达水平两两之间均存在显著的相关性(r=0.810,0.863,0.866,0.821,0.899,0.804,P<0.05)。TNF-α、TGF-β1、PDGF、CTGF的表达与胶原纤维含量(晚期放射性肝纤维化程度)呈显著的正相关性(r=0.892,0.848,0.942,0.764,P<0.05)。结论肝脏受照射剂量是导致放射性肝损伤的重要因素。TNF-α、TGF-β1、PDGF、CTGF在促进肝损伤的形成中可能发挥了重要作用;照射剂量与放射性肝损伤的发生密切相关。ObjectiveTo observe the relationship between histological changes of radiation-indueed liver injury and dosage effect, to find out a new way to treat liver cancer. MethodEstablishment of rabit model for semi-liver radiation-indueed liver injury, observed the expression of tumor necrosis factor-α (TNF-α), transforming growth factor-β1 (TGF-β1), platelet-derived growth factor (PDGF) and connective tissue growth factor (CTGF) in liver tissue caused by radiation and investigate the association between the cytokines and radiation-indueed liver injury and those factors which influence them. ResultAfter radiation, TNF-α, TGF-β1, PDGF, CTGF were significantly expressed in cytoplasm of hepatic fibrous septum, central veios and around portal area. With the radiation dose increased, expression levels of TNF-α, TGF-β1, PDGF and CTGF in the same material area of liver in different dose groups were increase, the differences were significant (P 〈 0.05), and the differences were significant with control group (P 〈 0.05); the expression levels of TNF-α, TGF-β1, PDGF and CTGF in different areas of the same radiation was different, the high dose area was more positive than low dose area, and the low dose area was more positive than non-radiation area, the differences were significant (P 〈 0.05), and the differences were significant with control group (P 〈 0.05). The expression levels of TNF-α, TGF-β1, PDGF and CTGF and the degree of hepatic fibrosis correlated with the irradiation dose positively (r = 0.899, 0.958, 0.959, 0.968, 0.967, P 〈 0.05), the expression levels of TNF-α, TGF-β1, PDGF, CTGF were positively correlated (r = 0.810, 0.863, 0.866, 0.821, 0.899, 0.804, P 〈 0.05). The expression levels of TNF-α, TGF-β1, PDGF and CTGF had positive correlation with the degree of fibrosis (r = 0.892, 0.848, 0.942, 0.764, P 〈 0.05).
