体位改变对胃复安在胶囊内镜消化道转运的影响

摘　　要：目的讨论分析患者接受肌注胃复安后不同体位对胶囊胃内停留时间的影响。方法对83例患者进行回顾性分析,所有患者均吞服胶囊并且1h后胶囊仍然停留在患者胃中,按照随机数字表法将所有患者分成三组,分成A组(n=27)、B组(n=28)、C组(n=28),参与本研究所有患者在服用胶囊之后均接受肌注胃复安,医护人员帮助不同组患者采取不同体位,其中A组患者为仰卧位、B组患者为右侧卧位、C组患者为坐立位,医护人员对不同组患者的胶囊胃停留时间情况进行观察并对比分析。结果相比于C组,A组和B组患者胶囊胃内停留时间均更短(P<0.05),但是A组和B组患者胶囊胃内停留时间和三组患者的小肠通过时间指标数据比较,差异无统计学意义(P>0.05)。结论对于吞服胶囊1h后仍然存在于胃中的患者,为了达到预期临床效果可以对其采取肌注胃复安用药,同时帮助患者采取右侧卧位休息或者仰卧位休息,能够从根本上缩短胶囊胃中停留时间,促进患者消化道运转,以此达到预期临床效果。Objective To discuss the effect of different postures after intramuscular metoclopramide on capsule retention time in the stomach. Methods The clinical data of 83 cases were retrospectively analyzed, and all patients swallowed capsules, and the capsule remained in the patient’s stomach after one hour. All cases were randomly divided into three groups, group A（n=27）, group B（n=28） and group C（n=28）. All cases received an intramuscular injection of metoclopramide after taking capsules. Doctors and nurses helped different groups of patients adopt different postures, group A patients in supine position, group B in right lateral position and group C in sitting position. Capsule gastric retention time was observed and analyzed. Results Compared with group C, capsule gastric retention time in the group A and the group B was shorter（P 〈0.05）; But, capsule gastric retention time between the group A with the group B, and intestinal transit time in the three groups showed no significant difference（P 〉0.05）. Conclusion In order to achieve the desired clinical effect, intramuscular metoclopramide for patients with capsule still in the stomach after one hour of taking capsule, and at the same time, assisting the patient with right lateral decubitus or supine position can radically shorten the retention time of capsules and promote the digestive tract functioning in patients.
