帕金森病患者快速眼动睡眠期行为障碍的多项睡眠图研究

摘　　要：目的：探讨帕金森病（PD）患者快速眼动睡眠（REM）期行为障碍（RBD）的不同诊断方法阳性率差异及对其病情进展、睡眠结构造成的影响。方法：共收集PD患者30例进行REM期行为障碍筛查量表（RBDSQ）评分和H—Y分期，并使用多项睡眠监测图（PSG）对所有入选的原发性PD患者睡眠过程进行监测，并分析RBD与PD病情严重程度、睡眠结构之间的相关性。结果：与无RBD组的PD患者相比，RBD组PD患者睡眠潜伏期缩短，REM比例增加。PSG诊断RBD的阳性率高于RBDSQ。结论：PSG监测对RBD诊断的阳性率较RBDSQ更高；本次研究中未发现RBD与PD病情严重程度的相关性；本研究中PD伴RBD患者较不伴RBD的PD患者入睡潜伏期缩短、REM期所占比例增加。Objective： To investigate the efficiency of different diagnostic methods for rapid eye movement sleep behavior disorder （ RBD）, the correlation between RBD and the duration of Parkinson＇s disease （PD） and the correlation between RBD and sleep structure. Methods： 30 patients with PD were enrolled in this study and were assessed by the eye movement sleep behavior disorder screening scale （RBDSQ） and H-Y stages. All the selected patients with PD were detected by polysomnography （PSG） during their sleep, and then the data were analyzed. Results.. The sleep latency of RBD group was longer than that of non-RBD group, and the percentage of REM of RBD group was higher than that of non-RBD group. The positive rate of PSG in the diagnosis of RBD was higher compared with RBDSQ. Conclusion.. The positive rate of PSG in the diagnosis of RBD is higher than that of RBDSQ. There is no significant correlation between RBD and PD severity in this study. Compared with that of PD patients without RBD, the sleep latency of PD patients with RBD is shorter and the percentage of rapid eye movement（REM） is larger.
