个体化单侧完全性唇裂修复术式分析

摘　　要：目的:分析单侧完全性唇裂的个体化修复术式。方法:选择600例单侧完全性唇裂患儿,A型(183例)采用Millard法,B型(152例)采用改良华西法,C型(177例)采用改良华西法,D型(88例)采用改良华西法或上下三角瓣结合法;术后随访6~48个月,平均20.4个月,评价手术效果。结果:所有患儿均顺利完成手术,切口均获得Ⅰ期愈合,无围手术期死亡,无感染征象,住院时间(6.8±1.4)d;瘢痕外观无异常增生达95.5%,白唇卷一致性比较完好、连续占85.7%,唇红缘一致性比较完好、连续占87.0%,丘比特弓形态完好占78.2%,健患侧唇高等高占88.2%,健患侧鼻孔对称占78.5%。结论:对于单侧完全性唇裂患儿,制定个体化的唇裂修复术式可获得较好的疗效。Objective：To analyze the individualized repair of unilateral complete cleftlip. Methods：600 cases of unilateral complete clefllip were selected. Type A （183 cases） was treated by the Millard, type B （152 cases） by the modified Huaxi, type C （177 cases） by the modified Huaxi, and type D （88 cases） by the modified Huaxi or triangular flap junction. The patients were followed up for 6 -48 months, average （20. 4 ± 7.4） months, and the effect of operation was evaluated. Results ： All the patients completed the operation successfully. All incisions were healed by first intention. There was no perioperative death, no sign of infection, and the length of stay was （6.77 ± 1.4 ） days. Up to 95.5 % of patients had no abnormal hyperplasia in scar appearance; continuous up to 85.7 % had relatively good white lips consistency; continuous up to 87. 0 % had intact red lip edge consistency; up to 78.2 % had good cupid bow shape; up to 88.2 % had healthy lateral lip; up to 78.5 % had the nose symmetry of the affected side. Conclusion： For the patients with unilateral complete cleftlip, individualized cleftlip repair can be used to achieve better results.
