摘  要:目的探讨健脾益智胶囊联合丰富康复训练对缺血性脑卒中大鼠运动功能的影响及可能作用机制。方法 72只SD大鼠随机分为假手术组、模型组、康复组、综合康复组,每组18只。除假手术组外其余各组采用热凝闭大脑中动脉法复制缺血性脑卒中大鼠模型。造模成功后第3天开始康复组给予丰富康复训练治疗,每日1次;综合康复组在康复组的基础上给予健脾益智胶囊混悬液15 ml/(kg·d)灌胃,每日1次。各组大鼠分别于造模成功后7、14、21天进行Longa评分、平衡木行走实验评分、转棒上行走实验评分,并检测脑组织梗死灶周围皮质神经生长相关蛋白43(GAP-43)表达。结果各时间点综合康复组Longa评分及平衡木行走实验评分、转棒上行走实验评分均较模型组降低,大鼠脑组织梗死灶周围皮质GAP-43阳性细胞数较模型组增加,且优于康复组(P<0.05或P<0.01)。结论健脾益智胶囊联合丰富康复训练的综合治疗方法可有效地促进缺血性脑卒中大鼠的神经功能恢复,其机制可能与增加大鼠脑组织梗死灶周围皮质GAP-43表达有关。Objective To explore the effect of Jiartpi Yizhi Capsule (健脾益智胶囊) combined with enriched rehabilitation training on the motor function of rats with ischemic stroke and its possible mechanism. Methods Seventy-two Sprague Dawley rats were randomly divided into a sham operation group, a model group, a rehabilitation group and a comprehensive rehabilitation group, with 18 rats in each group. All the groups except the sham operation group were treated by thermocoagulation on middle cerebral artery to replicate the rat model of ischemic stroke. Since the third day after successful modeling, both the rehabilitation group and the comprehensive rehabilitation group were given enriched rehabilitation training for treatment, once a day; and the comprehensive rehabilitation group was given 15ml/(kg · d) of Jianpi Yizhi Capsule suspension once a day in addition. Conducted Longa score, balance beam walking test score and walking rod test score were evaluated, and the expression of nerve growth associated protein 43 (GAP-43) in periinfarction cortex of brain tissue was detected in each group on the 7th, 14th and 21st days after modeling, respectively. Results At each time point, the Longa score, the balance beam walking test score and the walking rod test score in the comprehensive rehabilitation group were all lower than those in the model group. The number of GAP-d3 positive cells in periinfarction cortex of brain tissue of rats in the comprehensive rehabilitation group increased compared with that in the model group, and it was better than that in the rehabilitation group (P 〈 0. 05 or P 〈 0.01 ). Conclusion Jianpi Yizhi Capsule combined with enriched rehabilitation training could effec- tively promote the recovery of neurological function in rats with ischemic stroke, and its mechanism may be related to the increase of GAP-43 expression in periinfarction cortex of rat brain tissue.
