摘 要:目的调查住院肺癌患者营养状况,分析营养不良与体成分及血液指标的相关性。方法对连云港市第一人民医院自2016年3月至2017年5月的124例住院肺癌患者和105例肺非肿瘤性疾病患者应用患者主观整体评估进行营养评估,采用人体成分分析仪进行体成分分析,同时进行血液指标检测。结果 124例肺癌患者中,40.3%的肺癌患者存在可疑或中度营养不良(PG-SGA 2~8分),44.4%的肺癌患者存在重度营养不良(PG-SGA≥9分);肺癌患者与肺非肿瘤性疾病患者比较,营养不良的发生率更高,差异有统计学意义(χ~2=24.3,P<0.05);营养不良组患者体重、体质指数、骨骼肌、脂肪、血红蛋白、血清白蛋白、血清前白蛋白等指标低于非营养不良组(P<0.05),PG-SGA评分与体成分及血清白蛋白、血清前白蛋白呈负相关(P<0.05),PG-SGA评分与血红蛋白的相关性不具有统计学意义(P>0.05)。结论住院肺癌患者不良发生率高,PG-SGA评分与体成分及血清白蛋白、血清前白蛋白有良好的相关性。PG-SGA是评估肺癌患者营养不良简便、有效的工具,建议对肺癌患者进行包括PG-SGA在内的营养评估,为营养治疗提供依据。Objective To investigate the nutritional status of lung cancer patients, and to find the relationship of malnutrition,body composition and laboratory index. Methods From March 2016 to May 2017, we collect 124 patients with lung cancer and105 lung non-tumor patients in Lianyungang first people’s hospital, The nutritional status was evaluated by patient-generatedsubjective global assessment (PG-SGA), the body composition was measured by body composition analyzer and in the meantime,laboratory index were evaluated. Results Among the 124 lung patients, 40.3% of the lung cancer patients were suspicious ormoderate malnutrition and 44.4% of the lung cancer patients were severe malnutrition. The incidence of malnutrition was higher inlung cancer patients than in patients with lung non-tumor diseases, and the difference was statistically significant (χ2=24.3, P〈0.05).Body weight, body mass index, lean body mass, body fat mass, hemoglobin, albumin, prealbumin in malnutrition patients werelower than those in lung non-tumor patients (P〈0.05). The score of PG-SGA was inversely related to body composition index ,albumin and prealbumin (P〈0.05). The correlation between PG-SGA score and hemoglobin was not statistically significant (P〉0.05).Conclusions The incidence of malnutrition was high in lung cancer patients. The score of PG-SGA is negatively correlated withbody composition index, albumin and prealbumin. PG-SGA is a simple and effective tool to evaluate the malnutrition of lung cancerpatients. Nutritional assessment, including PG-SGA, is recommended for patients with lung cancer to provide evidence for nutritionaltreatment.
