不同手术治疗卵巢巧克力囊肿对患者后续超促排卵周期和生育功能影响的比较

摘　　要：目的:探讨不同手术治疗卵巢巧克力囊肿对患者后续超促排卵周期和生育功能影响。方法:选取2011年2月至2015年2月108例卵巢巧克力囊肿患者为研究对象,分成两组,研究组予腹腔镜下卵巢巧克力囊肿剥除术,对照组开腹治疗,观察不同方法治疗后对围术期指标、后续超促排卵周期和生育功能影响性。结果:研究组在术中出血量、住院时间、排气时间、镇痛率上明显低于对照组,两组比较差异显著(P<0.05),而研究组在手术时间上长于对照组(P<0.05);手术后6个月对照组FSH、LH较术前显著升高,E2水平显著下降,手术前后比较差异显著(P<0.05),研究组手术前后则无明显变化(P>0.05),术后6个月以上指标比较差异显著(P<0.05);对照组自然妊娠率32.56%、体外受精妊娠20.93%,研究组自然妊娠率56.82%、体外受精妊娠38.64%,两组比较差异显著(P<0.05);研究组在促性腺激素使用量、超促排卵周期时间上显著低于对照组,在获卵数、MⅡ卵泡数量上显著高于对照组,两组比较差异显著(P<0.05)。结论:腹腔镜下卵巢巧克力囊肿剥除术对有女性生育功能和后续超促排卵周期影响性小,对卵巢功能影响性低。Objectives： To investigate the effect of different surgical treatment on patients with ovarian chocolate cyst. Methods： 108 cases of ovarian chocolate cyst from February 2011 to February 2015 were selected as the research object,and divided into 2 groups. The study group were treated with laparoscopic enucleation of ovarian chocolate cyst,and control group received laparotomy treatment. The perioperative indexes,follow-up ovulatory cycle and fertility function of the two groups were observed. Results： The amount of bleeding during the operation,hospitalization time,exhaust time,analgesic rate of study group were significantly lower than these of control group,with significant differences（ P〈0. 05）,while the operation time in study group was longer than that of control group（ P〈0. 05）. At 6 months after surgery,FSH and LH in control group were significantly higher than these before operation. The level of E2 decreased significantly,and there was significant difference（ P〈0. 05）. There was no sig-nificant change in study group between before and after the operation（ P〈0. 05）. At more than 6 months,there were significant differences（ P〈0. 05）; the natural pregnancy rate of control group was 32. 56%,and IVF pregnancy rate was 20. 93%. The natural pregnancy rate of study group was 56. 82%,and IVF pregnancy rate was38. 64%,with significant differences between two groups（ P〈0. 05）. The gonadotropin dosage and superovulation cycle time of study group were significantly lower than these of control group,and the number of oocytes and M II follicles of study group was significantly higher than that of control group（ P〈0. 05）. Conclusion： Laparoscopic enucleation of ovarian chocolate cyst has little influence on the reproductive function and ovulatory cycle of female.
