摘 要:目的:总结黏蛋白家族成员MUC-1、MUC-2及MUC-5AC在肺恶性肿瘤中的表达,并分析它们在病理诊断中的意义。方法:回顾性分析中日友好医院72例肺恶性肿瘤患者,其中包括黏液性腺癌22例、非黏液性腺癌29例、其它原发性肺癌8例、消化道转移癌13例。观察这些病例的MUC-1、MUC-2、MUC-5AC蛋白表达情况,并与患者临床病理信息的关系进行统计分析。结果:MUC-1在非黏液性腺癌中的表达率高于黏液性腺癌(86.2%、54.5%);MUC-5AC在黏液性腺癌中的表达率(90.9%)高于其它类型肿瘤。来源于消化道的肺转移性癌高表达MUC-2(69.2%),肺原发肿瘤则较少表达MUC-2(13.6%)。黏蛋白MUC-2、MUC-5AC与患者的肿瘤来源存在相关性(P<0.05);黏蛋白MUC-1、MUC-2、MUC-5AC的表达与年龄、性别、吸烟史、有无淋巴结转移的相关性不具有统计学意义(P>0.05)。结论:黏蛋白家族成员MUC-1、MUC-2、MUC-5AC是很好的一组生物标志物,其免疫组织化学染色对肺恶性肿瘤的诊断与鉴别诊断具有一定价值。Objective:To summerize the expressions of mucin family members MUC-1,MUC-2 and MUC-5AC in lung malignancies,and to analyze the diagnostic significance of those expressions.Methods:We retrospectively reviewed 72 cases of lung malignancies in China-Japan Friendship Hospital,including 22 cases of mucinous adenocarcinoma,29 cases of non-mucinous adenocarcinoma,8 cases of other primary lung cancer and 13 cases of metastatic carcinoma from digestive tract.The correlation between MUC-1,MUC-2 and MUC-5AC expressions and clinic pathological characteristics of those cases were analyzed.Results:Positive rate of MUC-1was higher in non-mucinous adenocarcinomas(86.2%)than in mucinous adenocarcinomas(54.4%). MUC-5AC expression rate in mucinous adenocarcinomas(90.9%)was higher than that in other types of noeplasms.Tumors derived from digestive tracts(69.2%)expressed higher MUC-2 level than that origined from lungs(13.6%).The expression of MUC-2 and MUC-5AC were related to tumor origin(P〈 0.05).However,correlation between all the mucins' expressions and age,gender,smoking history and lymph node metastasis showed no statistical significance(all P 〉0.05).Conclusion:MUC-1,MUC-2 and MUC-5AC are a group of effective biomarkers,their expressions by immunohistochemistry have considerable value for diagnosis and differential diagnosis of lung malignancies.
