摘 要:目的:探讨最大脂肪氧化(maximal fat oxidation,FATmax)强度运动对超重/肥胖青年男子动脉硬度(arterial stiffness,AS)的影响。方法:选取32名无规律性运动习惯的超重/肥胖青年男子(BMI≥25kg/m2)为研究对象,随机分为运动组(n=16)和对照组(n=16)。运动组完成12周FATmax强度走跑运动(每周3~5次,每次40~60 min),对照组保持平常生活习惯,不从事任何规律性运动。12周前后分别进行身体成分、心肺适能(VO2max)和C反应蛋白(CRP)、血常规等血液指标测试,采用臂踝动脉脉搏波传导速度(brachial-ankle pulse wave velocity,ba PWV)评估动脉硬度变化情况。结果:与训练前相比,运动组受试者12周训练后的体重、BMI、体脂量、体脂百分比、ba PWV、CRP、血小板/淋巴细胞比值(PLR)和嗜中性粒细胞/淋巴细胞比值(NLR)等显著降低(P<0.05),VO2max和高密度脂蛋白胆固醇(HDL-C)显著升高(P<0.05)。训练前后ba PWV变化量(△ba PWV)分别与△CRP(r=0.604)、△NLR(r=0.535)、△WBC(r=0.406)呈正相关(P<0.05,n=32),与△VO2max呈负相关关系(r=-0.660,P<0.05,n=32)。结论:FATmax强度运动是下调机体炎症水平、提高心肺适能、改善血管功能的重要手段之一。有氧运动降低超重/肥胖青年男子动脉硬度的机制有待深入研究。Objective To assess the impact of maximal fat oxidation intensity(FATmax) exercise on arterial stiffness in overweight/obesity young men. Method Thirty two overweight/obesity young men(BMI≥25 kg/m2) without the habit of doing exercises,were divided into an exercise group(n=16) and a control group(n=16) randomly. The exercise group completed a 12-week walk/run(FATmax intensity,3-5 times/week,40-60 min/time) intervention,while the control group maintained normal life style with.out regular exercise. The body composition,cardiopulmonary function(VO2 max),C reactive proteins(CRP) and blood hemodynamic were measured,and the brachial-ankle pulse wave velocity(baPWV)was observed to assess the arterial stiffness before and after the 12-week intervention. Results Signifi.cant decrease was observed in the average weight,body mass index,body fat mass,body fat percentage,baPWV,CRP,platelet/lymphocyte ratio(PLR) and neutrophil/lymphocyte ratio(NLR)(P〈0.05),while signif.icant increase was found in the average VO2 max and HDL-C(P〈0.05) after the training. After the in.tervention,the change in baPWV(△) was positively associated with △CRP(r=0.604),△NLR(r=0.535)and △WBC(white blood count)(r=0.406)(P〈0.05,n=32),but negatively correlated with △VO2 max(r=.0.660,P〈0.05,n=32). Conclusion FATmax intensity aerobic exercise is effective to downregulate the in.flammation,and improve aerobic fitness and vascular function. However,the potential mechanism of arte.rial stiffness improvement following exercise needs further study.
