摘  要:目的探讨甲状腺结节良恶性应用三维彩色直方图鉴别诊断的临床价值。方法选取70例甲状腺结节患者,其中男性33例,女性37例;年龄22~78岁,平均年龄43.2岁。所有患者均给予三维彩色直方图血流定量分析,将检查结果与病理结果进行比较,探讨血流指数(FI)、血管指数(VI)、血管血流指数(VFI)鉴别诊断良恶性甲状腺结节的价值。结果通过数据重建处理均获得满意的三维能量多普勒图像。共检出结节78个,良性结节患者的VI、FI、VFI等指标均低于恶性结节患者,差异均具有显著统计学意义(P<0.01)。VI鉴别诊断甲状腺良恶性结节的曲线下面积为0.851,其诊断截值为0.07,VI≥0.07的鉴别诊断灵敏度为98.1%,特异度为72.6%。FI鉴别诊断的曲线下面积为0.829,诊断截值为22.67,FI≥22.67鉴别诊断灵敏度和特异度分别为81.4%和83.5%。VFI鉴别诊断良恶性结节的曲线下面积为0.827,诊断截值为1.86,VFI≥1.86的诊断灵敏度为88.2%,特异度为79.1%。结论三维彩色直方图有助于临床对甲状腺良恶性结节进行准确鉴别诊断,可将其血流定量分析作为良恶性甲状腺结节鉴别的重要依据。Objective To investigate the clinical value of differential diagnosis of benign and malignant thyroid nodules with three dimensional(3D) color histogram. Methods A total of 70 patients with thyroid nodules were enrolled, which included 33 males and 37 females, aged 22-78 years old with mean age of 43.2 years old. All of them were performed 3D color histogram, and examination results were compared with pathological results to investigate blood flow index(FI), vascular index(VI),blood flow index(VFI) in differential diagnosis of benign and malignant thyroid nodules. Results The 3D energy Doppler images were obtained by data reconstruction. A total of 78 nodules were detected. The VI FI, VFI and other indicators of benign nodules patients were lower than those of malignant nodules patients, the differences were statistically significant(P〈0.01). The benign and malignant in differential diagnosis of VI in thyroid nodules under curve area was 0.851, the diagnostic cutoff value was 0.07, the diagnosis sensitivity of VI ≥ 0.07 was 98.1 % and specificity was 72.6 %. The benign and malignant in differential diagnosis of FI in thyroid nodules under curve area was 0.829, the diagnostic cutoff value was 22.67, the diagnostic sen-sitivity of FI ≥ 22.67 was 81.4 % and specificity was 83.5 %. The benign and malignant in differential diagnosis of VFI in thyroid nodules under curve area was 0.827, the diagnostic cutoff value was 1.86, the diagnosis sensitivity of VFI ≥ 1.86 was 88.2 % and specificity was 79.1 %. Conclusion It is demonstrated that 3D color histogram is helpful for the differential diagnosis of benign and malignant thyroid nodules, and it could be used as an important basis for the differential diagnosis of benign and malignant thyroid nodules.
