摘  要:目的探讨三磷酸腺苷二钠氯化镁对宫内窘迫胎鼠肾脏的保护作用。方法将15只成年雌性SD孕鼠随机分为缺氧缺血再灌注组、假手术组和干预组,每组5只。于妊娠第19天以3.5%水合氯醛腹腔注射麻醉,缺氧缺血再灌注组和干预组通过无损伤动脉钳钳夹双侧子宫、卵巢动脉建立缺氧缺血宫内窘迫模型,缺氧20 min后拆除动脉夹以恢复血供,并逐层关腹。假手术组只开关腹,不钳夹子宫、卵巢动脉。干预组缺血前30 min给予三磷酸腺苷二钠氯化镁45 mg/kg腹腔注射,缺氧缺血再灌注组、假手术组于相同时间给予同等体积的0.9%氯化钠注射液腹腔注射。各组均于手术后24 h剖宫取胎,计算胎鼠死亡率。每组随机抽取10只胎鼠取肾组织制备标本,检测组织中丙二醛(MDA)水平,行HE染色高倍镜下观察胎鼠肾组织病理情况。结果缺氧缺血再灌注组胎鼠死亡率明显高于假手术组(P<0.05),干预组胎鼠死亡率明显低于缺氧缺血再灌注组(P<0.05)。缺氧缺血再灌注组胎鼠肾组织中MDA含量明显高于假手术组(P<0.05),干预组胎鼠肾组织中MDA含量明显低于缺氧缺血再灌注组(P<0.05)。干预组肾小管上皮细胞水肿样变性明显轻于缺氧缺血再灌注组。结论三磷酸腺苷二钠氯化镁能够减轻宫内窘迫胎鼠肾组织的损伤,对缺氧缺血肾细胞有保护作用。Objective It is to study the renal protection of triphosphate and magnesium chloride in intrauterine distress fetal rats. Methods 15 cases of adult female Sprague-Dawley pregnant rats were randomly divided into three groups: ischemic and hypoxia reperfusion group( I/R group),sham operation group( sham group),triphosphate and magnesium chloride treatment group( T/M group). Each group had 5 rats. On the 19 th day of gestation,all the rats were anesthetized by3. 5% chloral hydrate( 1 m L/100 g) by intraperitoneal injection. The models of ischemia and hypoxia and intrauterine distress were established by clamping uterine artery and ovarian artery with no-damaged artery forceps in I/R group and T/M group,after ischemia and hypoxia for 20 min,the artery forceps were removed to recovery blood supply,then the abdomen was closed layer by layer,only open and close of abdomen without clamping uterine artery and ovarian artery was performed in sham group. At 30 min before ischemia,T/M group was given adenosine triphosphate 45 mg/kg by intraperitoneal injection,sham operation group and I/R group were given the same volume of 0. 9% sodium chloride injection at the same time.The rats in every group were sacrificed in 24 h after operation to get fetal rats from uterine to calculate fetal mortality. 10 fetal rats in each group were randomly selected and their renal tissue was gotten to make exams to determine the level of malondialdehyde( MDA) and observe the pathological changes of fetal kidney under HE staining. Results The mortality of fetal rats in I/R group was significantly higher than that in sham operation group( P < 0. 05). The mortality of fetal rats in T/M group was significantly lower than that in I/R group( P < 0. 05). The content of MDA in I/R group was significantly higher than that in sham operation group( P < 0. 05),but the activity of MDA in T/M group was significantly lower than that in I/R group( P < 0. 05).
