视网膜光凝/冷凝对超声乳化IOL植入联合视网膜脱离复位术后囊袋收缩综合征的影响

摘　　要：目的:评估视网膜光凝/冷凝对白内障超声乳化人工晶状体植入联合视网膜脱离复位术后囊袋收缩综合征(CCS)的影响。方法:回顾性分析2014-01/2016-06确诊的孔源性视网膜脱离伴发年龄相关性白内障适合作气体填充的患者244例244眼,所有患者均行白内障超声乳化人工晶状体植入联合视网膜复位术,根据术中对视网膜裂孔实行冷凝或光凝将其分为两组。其中光凝组198眼,冷凝组46眼。于术后1d,1 wk,1、2、3、6mo,1 a观察视网膜复位及裂孔闭合情况,同时观察囊袋形状及人工晶状体的位置和术眼最佳矫正视力(BCVA),对两组数据进行统计学分析。结果:光凝组中发生晶状体前囊膜收缩综合征6眼(3.0%),冷凝组中发生5眼(10.9%)。两种不同视网膜裂孔处理方法与术后CCS的发生有相关性(OR:0.256,P=0.021,95%CI:0.075~0.880)。结论:白内障超声乳化人工晶状体植入联合玻璃体手术中光凝/冷凝处理与术后CCS的发生相关,光凝更有利于预防术后CCS。AIM:To compare the influence of endolaser or cryotherapy on capsule contraction syndrome(CCS)following phaco-vitrectomy and intraocular lens implantation in patients with rhegmatogenous retinal detachment.METHODS:A total of 244 eyes were enrolled in this retrospective analysis.The surgical approach was standard vitrectomy combined with phacoemulsification and intraocular lens(IOL) implantation simultaneously.There were 198 eyes underwent endolaser and 46 eyes underwent cryotherapy during the surgery.The main outcome measures were postoperative clinical outcomes including anatomical position of retina,the capsules and the position of IOLs and best corrected visual acuity(BCVA) at 1 d,1 wk,1,2,3,6 mo and 1 a.RESULTS:There were 6 eyes(3.0%) of CCS occurred in endolaser group and 5 eyes(10.9%) of CCS occurred in cryotherapy group.The different procedures during the surgery were significantly associated with the incidence ofCCS(odds ratio:0.256,P=0.021,95%CI:0.075-0.880). CONCLUSION:Different procedures during phaco-vitrectomy surgery are significantly associated with the incidence of CCS.Endolaser is more beneficial than cryotherapy in preventing CCS following phaco-vitrectomy and intraocular lens implantation.
