摘　　要：目的:探讨宫颈HPV感染与阴道内环境的相关性及阴道乳酸杆菌治疗在宫颈HPV感染的效果。方法:选取在某院就诊的宫颈HPV阳性300例、阴性300例患者作为研究对象。对比HPV阳性与阴性患者的阴道内环境及临床相关指标,分析HPV感染与阴道内环境的相关性,并将液基细胞学检查阴性、阴道镜检查未见异常的HPV阳性患者300例随机分为观察组150例与对照组150例,其中对照组患者进行定期随访,若存在阴道炎的接受常规阴道炎治疗(阴道塞药);观察组采用乳酸杆菌阴道胶囊阴道给药,比较两组患者半年及1年后的HPV转阴率。结果:其中对照组5例失访,观察组3例失访。HPV阳性患者与阴性患者在阴道内环境(pH值、菌群、BV)指标比较中存在显著差异,P<0.05;观察组治疗1年后HPV转阴率明显高于对照组,P<0.05。结论:pH值、菌群变化、细菌性阴道炎等阴道内环境的变化与宫颈HPV感染具有密切的相关性,而通过用乳酸杆菌阴道胶囊给药调整阴道内环境可有效提升患者HPV转阴率,具有临床应用及研究价值。Objective:To investigate the correlation between the infection of cervical HPV infection and intravaginal environment.Methods:300 cases of cervical HPV positive and 300 cases of HPV negative in a hospital were selected as the subjects.The vaginal environment and clinical related indexes of HPV positive and negative patients were compared,the correlation between HPV infection and vaginal environment was analyzed,and 300 patients with HPV positive which the liquid-based cytology was negative and colposcopy was not abnormal were randomly divided into observation group(n=150)and control group(n=150).The patients in the control group were followed up regularly,and if there was vaginitis,they were treated with routine vaginitis(vaginal suppository).The observation group was treated with lactobacillus vaginalis capsule vaginal administration.The negative rate of HPV after six months and one year in the two groups was compared.Results:Among them,5 cases were lost in the control group and 3 cases in the observation group.There were significant differences between the HPV positive patients and the negative patients in the vaginal environmental indicators(pH value,BV)(P<0.05);The negative rate of HPV after one year of treatment in the observation group was significantly higher than that in the control group(P<0.05).Conclusion:The changes of intravaginal environment such as pH,flora,and bacterial vaginitis were closely related to cervical HPV infection,and adjusting the vaginal environment with lactobacillus vaginalis capsule can effectively improve the HPV negative rate of patients,which has clinical application and research value.
