中性粒细胞体积分布宽度在溃疡性结肠炎中临床价值研究

摘　　要：目的探讨中性粒细胞体积分布宽度(NVDW)在溃疡性结肠炎(UC)诊断的价值。方法选取明确为UC的患者作为疾病组,按疾病活动指数将疾病组分为缓解期、活动期(轻、中、重度组),选取同期健康体检人群80例作为对照。运用LH750全自动血细胞分析仪检测中性粒细胞平均体积(MNV)、中性粒细胞体积分布宽度(NVDW),用ELISA法检测血清C-反应蛋白(CRP)水平。比较各组间指标的差异,并利用ROC曲线进行分析。结果与对照组比较,疾病组MNV、NVDW及血清中CRP水平明显升高,两者间差异有统计学意义(P<0.05);与缓解期比较,疾病组活动期MNV、NVDW、CRP水平显著增高(P<0.05),且其水平随溃疡性结肠炎患者病情严重程度增加而增高(P<0.05)。MNV、NVDW、CRP的ROC曲线下面积分别为0.928、0.982、0.875,灵敏度分别为0.862、0.897、0.828,特异度分别为0.872、0.925、0.775。结论 MNV、NVDW、CRP可以作为评估UC炎症严重程度的重要指标,其对UC具有重要诊断价值,MNV、NVDW的诊断价值高于CPR。Objective To investigate the value of neutrophil volume distribution width(NVDW)in the diagnosis of ulcerative colitis(UC).Methods Selected UC patients as the disease group,and divided into remission group and activity group(mild group,moderate group and severe group) according to disease activity index. Selected 80 healthy people as control group. Detected the mean neutrophil volume(MNV) and neutrophil volume distribution width(NVDW) by LH750 automatic hematology analyzer, and detected the level of serum C-reactive protein(CRP) by ELISA.Compared the difference of the indexes of each group,and analyzed by ROC curve. Results Compared with the control group, the levels of NVDW,MNV and CRP in disease group were significantly increased,the difference was statistically significant(P<0.05). Compared with the remission group, the levels of NVDW, MNV and CRP in activity group were significantly higher(P<0.05), and the levels increased with the severity of UC(P<0.05). The area under the ROC curve of MNV, NVDW and CRP was 0.928, 0.982 and 0.875, respectively. The sensitivity was 0.862, 0.897,0.828, and the specificity was 0.872, 0.925, 0.775, respectively. Conclusion MNV, NVDW and CRP can be used as an important index to evaluate the severity of UC inflammation,which has important diagnostic value for UC, and the diagnostic value of MNV and NVDW were higher than CPR.
