Effect and Influencing Factor of Photodynamic Therapy in Cervical Intraepithelial Neoplasia with Associated Human Papillomavirus Infection

摘  要:目的:探讨光动力疗法(PDT)治疗宫颈上皮内瘤样病变(CIN)和高危型HPV感染的效果及影响其疗效的因素。方法:将确诊为CINⅠ-Ⅱ合并高危型HPV感染的168名初治患者按照敷药时间和照光时间随机分为1组(敷药时间3 h)和2组(敷药时间5 h)/A组(照光时间30 min)和B组(照光时间45 min),每个患者进行PDT治疗3次及治疗后1,3,6个月复查HPV感染和宫颈病理情况。结果:CIN的治愈率为100%,HPV转阴率为70%。治疗后3个月与6个月复查,CIN的治愈率仍为100%,HPV的治愈率为69%。HPV转阴率在CINⅠ和CINⅡ患者之间、A组和B组患者间无统计学差异,2组患者的转阴率明显高于1组患者(77.8%vs 63.2%,P<0.05)。结论:PDT治疗CIN的治疗效果好,HPV清除率高达70%,且敷药时间5 h者HPV转阴率优于敷药3 h者。Objective:To investigate effect and influencing factor of photodynamic therapy in cervical intraepithelial neoplasia(CIN)with associated human papillomavirus(HPV)infection. Methods:168 patients with CIN Ⅰor Ⅱ with high-risk HPV infection were involved in the study. They were divided into group1 and group 2/group A and group B according to interval hour and irradiation time. All the cases were given photodynamic therapy with 5-ALA 3 times and follow-up of therapeutic effect. Results:After 1 month of end of treatment,the cure rate of CIN was 100%,HPV clearance rate was 70%. After 3 and 6 months of end of treatment,the cure rate of CIN was 100%,HPV clearance rate was 69%.There were no statistically significance of HPV clearance rate between patients with CIN Ⅰ and CIN Ⅱ,and between patients of A and B group. But HPV clearance rate was higher in group 2(77.8%)than in group 1(63.2%). Conclusion:There are satisfactory results of photodynamic therapy in cervical intraepithelial neoplasia and human papillomavirus infection. A influence factor is interval of dressing and irradiation. Effect in patients with interval 5 h is better than those with interval 3 h.
