摘  要:燕树勋教授系国家中医药管理局首批中医学术流派-湖北陈氏中医瘿病(甲状腺病)学术流派主要传承人,从事中医瘿病学临床、科研和教学工作20余年,具有深厚的中医理论知识和丰富的临床经验,尤其对甲状腺癌的诊疗有独到之处。甲状腺癌和甲状腺癌术后均属于中医"瘿瘤""石瘿"的范畴,燕教授认为甲状腺癌术后的病因有肝气郁结、饮食失调、起居失常、体质因素、水土因素、手术损伤、碘及激素治疗等,病机为本虚标实,标实以气郁、痰阻、血瘀、热毒为主,本虚包括气血阴阳俱虚。其中主要常见病机为气郁、痰阻,临床症候以气郁痰阻多见,治疗从痰气论治,以理气化痰为主,采用柴胡疏肝散合二陈汤加减,取得了满意的临床疗效。Prof. Yan Shuxun is the inheritor of the academic school for the treatment of goiter,approved by the State Administration of Traditional Chinese Medicine. Prof. Yan has been engaged in clinical practice,scientific research and education of goiter in TCM for over 20 years and has accumulated rich experiences in clinical work and TCM theories,especially for the unique treatment of goiter. Thyroid cancer and the postoperative condition of it are in the category of yingliu or shiliu in TCM. Prof. Yan believes that the postoperative disorder of goiter is related to liver qi stagnation,improper diet and living habit,constitutional factors,geographic factors,surgical injury,iodine and hormone treatment in etiology. The pathogenesis refers to the deficiency of the root course and excess of the secondary. The excess of the secondary indicates qi stagnation,phlegm obstruction,blood stagnation and heat toxin. The deficiency of the root cause refers to deficiency of qi,blood,yin and yang. Qi stagnation and phlegm obstruction are the most common in clinic,which should be treated with modified chaihu shugan san and er chen tang on the base of phlegm qi theory by regulating qi and resolving phlegm. The satisfactory clinical therapeutic effects are achieved.
