ZNF146在乳腺导管内癌肌上皮细胞中的表达及临床意义

摘　　要：目的检测锌指蛋白146(ZNF146)在不同级别乳腺导管内癌肌上皮细胞中的表达,探讨其临床病理诊断意义。方法收集74例乳腺组织标本,包括10例正常乳腺组织,64例乳腺导管内癌(其中低级别导管内癌6例、中级别导管内癌32例、高级别导管内癌26例)。采用免疫组化En Vision两步法,检测ZNF146在各种级别乳腺导管内癌肌上皮细胞中的表达情况,并与肌上皮标记物CK5/6进行比较。结果 ZNF146在不同级别的乳腺导管内癌肌上皮细胞有不同程度的表达,并且表达强度存在差异。低级别导管内癌阳性率100%(6/6),6例均呈强(+);中级别导管内癌阳性率43.8%(14/32),10例呈中等(+),4例呈弱(+);高级别导管内癌阳性率30.8%(8/26),8例均呈弱(+);各级别乳腺导管内癌肌上皮细胞阳性率差异显著(P<0.05)。同时,在乳腺导管内癌肌上皮细胞ZNF146和CK5/6阳性率为分别为43.8%和92.2%,两标记物阳性率比较差异显著(P<0.05)。结论 ZNF146是一个新的乳腺肌上皮细胞标记,在乳腺导管内癌的诊断中价值有限,其下调可能与乳腺导管内癌进展相关。Purpose To detect the expression of zinc finger protein 146（ ZNF146） in myoepithelial cells within different grades of breast intraductal carcinoma,and to explore the clinicopathological diagnosis significance. Methods 74 cases of breast specimens were collected,including 10 cases of normal breast tissue and 64 cases of breast intraductal carcinoma（ including 6 cases of low grade intraductal carcinoma,32 cases of intermediated grade intraductal carcinoma and 26 cases of high grade intraductal carcinoma）. ZNF146 expression in myoepithelial cells of different grades breast intraductal carcinoma were detected by immunohistochemistry using En Vision two steps method,and compared with expression of myoepithelial marker CK5/6. Results ZNF146 expression in myoepithelial cells of different grades breast intraductal cancer were various in both positive rate and staining intensity. The positive rate was 100%（ 6/6） in low grade intraductal carcinoma and all 6 cases showed strong staining; The positive rate was 43. 8%（ 14/32） in intermediated grade intraductal carcinoma,which included moderate staining of 10 cases and weak staining of 4 cases; The positive rate was 30. 8%（ 8/26） in high grade intraductal carcinoma which included weak expression of all 8 cases. The differences of positive rate in myoepithelial cells among different grade breast intraductal carcinoma were statistically significant（ P 0. 05）. Meanwhile,the positive rates of ZNF146 and CK5/6 in myoepithelial cells of breast intraductal carcinoma were 43. 8% and 92. 2%respectively and the difference betweent them was statistically significant（ P 0. 05）. Conclusion ZNF146 is one of new markers in breast myoepithelial cells with limited diagnostic value in the diagnosis of breast intraductal carcinoma,and its down-reduction may be related to the progression of breast intrductal carcinoma.
